DATE/TIME
TYPE
OCCUPANCY

AUTHORIZATION APPROVAL
04/12/99 16:41 PERMIT NO
SP STATUS
R-3 ISSUED
TO EXPIRE
PAGE

99-02548
ISSUED
04/12/1999
04/12/2000
2

AUTHORIZATION APPROVAL

This Authorization Notice is for an existing system and an inspection of the system
determined that it appeared adequate to serve the purpose proposed in the application. No
evidence of failure was found, and the system appeared to be functioning in a satisfactory manner,
The septic tank was pumped 05/08/98 . by CARLS SEPTIC and was inspected at that time,

This notice is issued with the recommendation that the septic tank be pumped and inspected by
a licensed septic tank pumper approximately every five years.

This notice does not guarantee satisfactory or continuous operation of the existing on-site
sewage disposal system. Planning approval may be required for the proposed development of the
property,

CUMMENIS AND CUNDII1DNS

1, CALL BEFORE YOU DIG .... IT'S THE LAW.
ATTENTION: Oregon law requires you to follow rules adopted
by the Oregon Utility Notification Center. Those rules are
set forth in OAR 952-001-0010 through OAR 952-001-0090. You
may obtain copies of the rules by calling the center.
(Note: The telephone number for the Oregon Utility Notifica-
tion Center is (503) 222~1987).
2, THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON APRIL 9,
1999. THE REVIEW IS TO CONNECT A TWO BEDROOM RESIDENCE TO
THE EXISTING SEPTIC SYSTEM. OUR RECORDS INDICATE THAT THE
SYSTEM WAS INSTALLED FEBRUARY 22, 1973. IT INCLUDES A 750
STEEL TANK AND 150' DF DRAINLINE. THE SHED SHOULD BE MOVED
OFF OF THE SEPTIC TANK AND THE DRIVEWAY MUST BE RE-ROUTED
CLEAR OF THE EXISTING DP~qINFIELD LINES. THE HOUSE SITE MAY
NEED TO BE SHIFTED TO THE WEST TO PROVIDE SPACE FOR A FULL
FUTURE DRAINFIELD REPLACEMENT AREA. WITH THOSE ADJUSTHENTS
WE HAVE NO OBJECTION TO THE PROPOSED CONNECTION~ THE SEPTIC
TANK SHOULD BE PUMPED ON A REGULAR SCHEDULE; ABOUT EVERY
FOUR YEARS TO EXTEND THE SYSTEM'S USE AS LONG AS POSSIBLE,
3, THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any
vehicular traffic, livestock and farming activities. Other
activities such as logging, excavating or filling, driveway
placement or any similar activities may also damage the soil
in the area and adversely affect the septic system approval.
The areas of the initial septic system and the future repair
system must both be protected,
