No evidence of failure was found and the system appeared to be <br />functioning in a satisfactory manner. The septic tank was pumped and inspected on 03/26/99 <br />by VALLEY SEPTIC <br /> <br />This notice is issued with the recommendation that the septic tank be pumped and inspected by <br />a licensed septic tank pumper approximately every five years. This notice does not guarantee <br />satisfactory or continuous operation of the existing on-site sewage disposal system, <br /> <br />Planning approval may also be required for any proposed development of the property, <br /> <br />COMMENTS AND CONDITIONS <br /> <br /> 1. CALL BEFORE YOU DIG .... IT'S THE LAW. <br /> ATTENTION: Oregon law requires you to follow rules adopted <br /> by the Oregon Utility Notification Center. Those rules are <br /> set forth in OAR 952-001-0010 through OAR 952-001-0090, You <br /> may obtain copies of the rules by calling the center. <br /> (Note: The telephone number for the Oregon Utility Notifica- <br /> tion Center is (503) 232-1987). <br />2, THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON APRIL 14, <br /> 1999. THIS APPLICATION IS TO REPLACE THE TWO BEDROOM HOUSE <br /> WITH A NEW THREE BEDROOM MANUFACTURED HOME. NO APPARENT CON- <br /> FLICT OR FAILURE WAS NOTICED, IT DOES APPEAR THAT THE DRAIN- <br /> FIELD IS SOMEWHAT LESS THAN 100 FEET FROM THE WELL AND THAT <br /> THE DP~qINFIELD IS ENCUMBERED BY THE DRIVEWAY AND SHOP. WE <br /> HAVE NO RECORD OF THE EXISTING SYSTEM IN OUR FILES, SPACE IS <br /> AVAILABLE FOR FUTURE REPAIRS. AN ALTERNATIVE SAND FILTER IS <br /> PROBABLY NECESSARY DUE TO THE SHALLOW REGIONAL WATER TABLE. <br /> THE SEPTIC TANK SHOULD BE PUMPED ABOUT EVERY FOUR TO FIVE <br /> YEARS TO EXTEND THE SYSTEM'S USEFUL SERVICE AS LONG AS <br /> POSSIBLE. OKAY FOR THE PROPOSED THREE BEDROOM RESIDENCE. <br />3. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities. Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval. <br /> The areas of the initial septic system and the future repair <br /> system must both be protected. <br /> <br /> <br />
