The approval is site specific to the area tested, and the septic <br />system must be located in the immediate area of the test pits, unless otherwise authorized <br />by this office. A new site evaluation may be required if the system is proposed for a <br />different area on the property. <br /> <br />This approval was given on the basis that the lot or parcel will not be further partitioned <br />or subdivided and that conditions on this property and on adjacent properties are not altered <br />in any manner which would prohibit the issuance of a permit. Soils in the approved area <br />shall not be altered by cutting, filling, mr other developmental activities which would <br />impede the successful operation of the initial or replacement absorption systems. Failure <br />to comply with the above items may void this approval. <br /> <br />The site evaluation approval runs with the land, but the permit is non-transferable and non- <br />refundable. <br /> <br />Keep this document with your records. Show this permit to the septic installer prior to <br />installation of the system. M~J<IMUM <br /> TOTAL DRAIN MEDIA DEPTH~ 12INCHES <br /> DRAIN MEDIA UNDER PIPE: 6INCHES <br /> MINIMUM SOIL BACKFILL: 12INCHES <br /> GROUND WATER INTERC£PTOR: N <br /> <br /> The disposal trenches are constructed two (2) feet wide and on minimum ten (10) foot centers <br /> and with no line exceeding 125 feet in length. INSTALL THE SEPTIC SYSTEM AS CURRENTLY FIELD STAKED <br /> FOR LOCATION~ INSTALL A MINIMUM OF 100 FEET OF DPJ~INFIELD <br /> FOR THE INITIAL STANDARD SYSTEM. AN ALTERNATIVE SAND FILTER <br /> SYSTEM IS NECESSARY FOR FUTURE REPAIRS. REFER TO THE SITE <br /> PLAN OF THE STAKEOUT DATED MAY 19. 1999. OBSERVE ALL USUAL <br /> SETBACK AND SEPARATION DISTANCES. NOTE. ESPECIALLY THE 100 <br /> FEET MINIMUM SETBACK FROM THE POND AND DRAINAGEWAY. <br />3, THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection. The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />4.
