An inspection of the field determined <br />that it appeared adequate to serve the purpose proposed in the application, No evidence of <br />failure was found in the drainfield and the field appeared to be functioning in a satisfactory <br />manner, <br /> <br />The Minor Repair Permit is for replacement of the existing septic tank and inspection of the <br />property determined that a new tank, may be installed to replace the existing tank. The <br />following specifications shall apply to the installation of the septic tank: <br /> <br />MINIMUM SEPTIC TANK CAPACITY: 1500 <br /> <br />Call for inspection of the new septic tank after the tank has been installed and prior <br />to backfill, This permit is issued with the recommendation that the septic tank be <br />pumped and inspected approximately every five (5) years. <br /> <br />Keep this document with your records, Give a copy of this permit to the septic system <br />installer prior to the installation of the system, Any deviation from the approved plan <br />must have prior approval from this office. <br /> <br />COMMENTS AND CONDITIONS <br /> <br />1, CALL BEFORE YOU DIG .... IT'S THE LAW, <br /> A1-FENTION: Oregon law requires you to follow rules adopted <br /> by the Oregon Utility Notification Center. Those rules are <br /> set forth in OAR 952-001-0010 through OAR 952-001-0090. You <br /> may obtain copies of the rules by calling the center, <br /> (Note: The telephone number for the Oregon Utility Notifica- <br /> tion Center is (503) 222-1987), <br />2, THIS SEPTIC REVIEW WAS COMPLETED BY Z,EWING ON 4/21/99. THE <br /> EXISTING SEPTIC TANK IS LOCATED WITHIN THE ORIGINAL GAP~AGE <br /> ADDITION AND IS SLOWLY COLLAPSING, THE CURRENT RESTAURANT <br /> HAS A CAPACITY OF 18 SEATS WITH 2 EMPLOYEES, THE TOTAL FLOW <br /> FROM THIS FACILITY IS CALCULATED AT 750 GALLONS PER DAY. A <br /> 1125 GAL, SEPTIC TANK MIN, REQUIRED (1500 RECOMMENDED), A <br /> EFFLUENT FILTER IS REQUIRED ON ALL COMMERCIAL SEPTIC TANKS. <br /> THE OLD TANK MUST BE PUMPED AND ABANDONED. AN ABANDONMENT <br /> FORM AND PUMPING RECEIPT SUBMITTED AT THE TIME OF NEW TANK <br /> INSTALLATION, <br />2, THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection, The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site, <br />4. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities, Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any simiIar activities may also damage the soil <br /> in the area and adversely affect the septic system approval, <br /> The areas of the initial septic system and the future repair <br /> system must both be protected, <br />5. THE ATTACHED SEPTIC TANK ABANDONMENT FORM MUST BE COMPLETED <br /> AND RETURNED TO OUR OFFICE. The septic system installer <br /> must complete and sign the form and attach a copy of the <br /> receipt from the septic tank pumper for pumping the tank, <br /> <br /> <br />
