This may be obtained at the Marion County Assessor Office. <br /> Map Room, Fifth Floor, Franklin Building, 495 State Street, Salem. <br /> <br /> ] The existing septic tank must be pumped by a DEQ licensed pumping contractor and <br /> verification of pumping submitted to this office (see attached "Pumper Form"), <br /> <br /> ] Drainfields must be staked according to the attached. "Stakeout Procedure Guidelines," <br /> <br /> ]Easements must be previewed and approved by this office prior to recording. A <br /> copy of the recorded easement must be submitted prior to permit issuance. <br /> <br />[ ] The septic tank must be abandoned and the attached, "Septic Tank Abandonment" <br /> form submitted prior to permit issuance. <br /> <br /> ] The well must be abandoned and the attached. "Well Abandonment" form submitted. <br /> <br /> ] Two (2) detailed plans for the construction of a Sand Filter System must bn~ <br /> submitted (see attached), ~ <br />[~'~ Other ~f~_~)! ~ ~~ ~_~~ <br /> <br />This application will remain in the review process until 10/31/1999 pending submittal <br />of the above, At that time, the application will expire and fees will be forfeited. <br />If there are any questions, please do not hesitate to call.
