No <br />evidence of failure was found, and the system appeared to be functioning in a satisfactory manner. <br />The septic tank was pumped 5 /5 /99 , by MIKES SEPTIC and was inspected at that time. <br /> <br /> This notice is issued with the recommendation that the septic tank be pumped and inspected by <br />a licensed septic tank pumper approximately every five years. <br /> <br /> This notice does not guarantee satisfactory or continuous operation of the existing on-site <br />sewage disposal system. Planning approval may be required for the proposed development of the <br />property. <br /> <br />COMMENIS AND CDNDIIIUNS <br /> <br />1. CALL BEFORE YOU DIG .... IT'S THE LAW. <br /> A~ENTION: Oregon law requires you to follow rules adopted <br /> by the Oregon Utility Notification Center, Those rules are <br /> set forth in OAR 952-001-0010 through OAR 952-001-0090. You <br /> may obtain copies of the rules by calling the center, <br /> (Note: The telephone number for the Oregon Utility Notifica- <br /> tion Center is (502) 232-1987), <br />2. THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON MAY 21, <br /> 1999. THE REVIEW IS TO CONSIDER REPLACING THE THREE BEDROOM <br /> MOBILE HOME WITH A SLIGHTLY LARGER THREE BEDROOM MANUFACT- <br /> URED HOME. NO APPARENT CONFLICT OR FAILURE WAS NOTICED, THE <br /> NEW HOME MUST MEET A FIVE FEET MINIMUM S~TBACK FROM THE TANK <br /> AND TEN FEET MINIMUM FROM THE DP~qINFIELD. OUR RECORDS IN- <br /> DICATE THAT THE EXISTING SEPTIC SYSTEM WAS INSTALLED APRIL <br /> 25. 1975. IT INCLUDES A 1000 GALLON CONCRETE TANK AND 450' <br /> OF DRAINFIELD. WE HAVE NO OBJECTION TO THE REPLACEMENT UNIT. <br /> THE SEPTIC TANK SHOULD BE PUMPED ON A REGULAR SCHEDULE, <br /> ABOUT EVERY FOUR TO FIVE YEARS TO EXTEND THE SYSTEM'S USEFUL <br /> SERVICE AS LONG AS POSSIBLE. <br />2. THE A~ACHED OPJ~NGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection. The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />4, THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities. Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval. <br /> The areas of the initial septic system and the future repair <br /> system must both be protected, <br /> <br /> <br />
