DATE/TIME
TYPE
OCCUPANCY

EXISTING SYSTEM APPROVAL
12/23/99 16:45 PERMIT NO
SPMS STATUS
R-3 ISSUED
TO EXPIRE
PAGE

99-05256
ISSUED
12/23/1999
12/22/2000
2

EXISTING SYSTEM APPROVAL

A site inspection was made to determine the condition of the existing on-site sewage system
located on the property, No evidence of Failure was found and the system appeared to be
functioning in a satisfactory manner.

This notice is issued with the recommendation that the septic tank be pumped and inspected by
a licensed septic tank pumper approximately every five years, This notice does not guarantee
satisfactory or continuous operation of the existing on-site sewage disposal system.

Planning approval may also be required for any proposed development of the property.

COMMENTS AND CONDITIONS

1. CALL BEFORE YOU DIG .... IT'S THE LAW.
ATTENTION: Oregon law requires you to follow rules adopted
by the Oregon Utility Notification Center. Those rules are
set forth in OAR 952-001-0010 through OAR 952-001-0090. You
may obtain copies of the rules by calling the center.
(Note: The telephone number for the Oregon Utility Notifica-
tion Center is (502) 232-1987).
2, THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON DECEMBER
15, 1999, THE REVIEW WAS TO CHECK FOR CONFLICTS BETWEEN THE
SEPTIC SYSTEM REQUIREMENTS AND RECENT CONSTRUCTION. DURING
THE ON-SITE MEETING WITH DICK BRAY AND ZAN EWING WE DID FIND
THAT A PORTION OF THE DECK IS BUILT OVER THE SEPTIC TANK AND
THE EXTENDED ROCKED DRIVEWAY DOES ENCUMBER PART TO THE DRAIN
FIELD, THE SEPTIC SYSTEM WAS FUNCTIONING NORMALLY WITHOUT
EVIDENCE OF FAILURE, DICK INDICATES THAT PART OF THE DECK
CAN BE REMOVED IF NECESSARY TO PUMP THE SEPTIC TANK, SPACE
IS AVAILABLE TO CONSTRUCT AN ALTERNATIVE SAND FILTER SYSTEM
FOR FUTURE REPAIRS. WE RECOMMEND THAT THE SEPTIC TANK SHOULD
BE PUMPED ABOUT EVERY 5 YEARS TO EXTEND THE SYSTEMS SERVICE.
3. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any
vehicular traffic, livestock and farming activities, Other
activities such as logging, excavating or filling, driveway
placement or any similar activities may also damage the soil
in the area and adversely affect the septic system approval,
The areas of the initial septic system and the future repair
system must both be protected.
4. The septic tank should be pumped and inspected by a licensed
septic tank pumper approximately every five (5) years. The
septic tank is a reservoir for the solid materials in the
sewage waste stream. Excessive accumulation of these solid
materials in the septic tank will cause the solids to wash
out of the tank and into the drainfiled. This will cause
irreversible damage to the drainfield and will eventually
lead to a system failure.
