DATE/TIME : 09/28/99 11:54 TYPE : SP MINOR REPAIR SEPTIC PERMIT PERMIT NO STATUS ISSUED TO EXPIRE PAGE 99-08043 ISSUED 09/28/1999 09/27/2000 2 MINOR SEPTIC REPAIR PERMIT This property has been evaluated for a septic system repair permit for feasibility to repair an existing septic system for a RESIDENTIAL building, An inspection of the field determined that it appeared adequate to serve the purpose proposed in the application, No evidence of failure was found in the drainfield and the field appeared to be functioning in a satisfactory manner. The Minor Repair Permit is for replacement of the existing septic tank and inspection of the property determined that a new tank, may be installed to replace the existing tank. The following specifications shall apply to the installation of the septic tank: MINIMUM SEPTIC TANK CAPACITY: 1000 Call for inspection of the new septic tank after the tank has been installed and prior to backfill. This permit is issued with the recommendation that the septic tank be pumped and inspected approximately every five (5) years, Keep this document with your records. Give a copy of this permit to the septic system installer prior to the installation of the system. Any deviation from the approved plan must have prior approval from this office. Those rules are set forth in OAR 952-001-0010 through OAR 952-001-0090. You may obtain copies of the rules by calling the center, (Note: The telephone number for the Oregon Utility Notifica tion Center is (503) 232-1987). 2, THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON SEPTEMBER 28, 1999, THE REVIEW IS TO REPLACE THE DETERIORATED SEPTIC TANK WITH A NEW 1000 GALLON TANK, REFER TO THE SITE PLAN DATED SEPTEMBER 28, 1999. THE OLD TANK MUST BE PUMPED AND PROPERLY ABANDONED. SUBMIT A COMPLETED ABANDONMENT FORM TO OUR OFFICE UPON CONNECTION TO THE NEW TANK. 3, THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD prior to requesting an inspection, The card must be located near the driveway to the property to assist the inspector in locating the access onto the site. 4. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities. Other activities such as logging, excavating or filling, driveway placement or any similar activities may also damage the soil in the area and adversely affect the septic system approval, The areas of the initial septic system and the future repair system must both be protected. 5. OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules, The well placement is especially critical and must be located at least one-hundred (100) feet from the approved initial and replacement drainfield areas. A copy of the DEQ setback requirements is available in our office. 6. THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL When work is complete and ready for inspection, call our Automated Inspection Request line at 373-4427 to request an inspection, Enter the number "801" for a PRE-COVER inspec- tion. You may leave a message for the inspector if desired. Be sure the System Record form with the As-Built drawing is on the job or in our office when the inspection is requested An inspection should be completed within seven (7) days.
