The <br />following specifications shall apply to the installation of the septic tank: <br /> <br />MINIMUM SEPTIC TANK CAPACITY: 1000 <br /> <br />Call for inspection of the new septic tank after the tank has been installed and prior <br />to backfill. This permit is issued with the recommendation that the septic tank be <br />pumped and inspected approximately every five (5) years, <br /> <br />Keep this document with your records. Give a copy of this permit to the septic system <br />installer prior to the installation of the system. Any deviation from the approved plan <br />must have prior approval from this office. <br /> <br />COMMENTS AND CONDITIONS <br /> <br />1, CALL BEFORE YOU DIG .... IT'S THE LAW, <br /> ATTENTION: Oregon law requires you to follow rules adopted <br /> by the Oregon Utility Notification Center. Those rules are <br /> set forth in OAR 952-001-0010 through OAR 952-001-0090. You <br /> may obtain copies of the rules by calling the center, <br /> (Note: The telephone number for the Oregon Utility Notifica <br /> tion Center is (503) 232-1987). <br />2, THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON SEPTEMBER <br /> 28, 1999, THE REVIEW IS TO REPLACE THE DETERIORATED SEPTIC <br /> TANK WITH A NEW 1000 GALLON TANK, REFER TO THE SITE PLAN <br /> DATED SEPTEMBER 28, 1999. THE OLD TANK MUST BE PUMPED AND <br /> PROPERLY ABANDONED. SUBMIT A COMPLETED ABANDONMENT FORM TO <br /> OUR OFFICE UPON CONNECTION TO THE NEW TANK. <br />3, THE ATTACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection, The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />4. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities. Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> in the area and adversely affect the septic system approval, <br /> The areas of the initial septic system and the future repair <br /> system must both be protected. <br />5. OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by <br /> the DEQ rules, The well placement is especially critical <br /> and must be located at least one-hundred (100) feet from the <br /> approved initial and replacement drainfield areas. A copy <br /> of the DEQ setback requirements is available in our office. <br />6. THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL <br /> When work is complete and ready for inspection, call our <br /> Automated Inspection Request line at 373-4427 to request an <br /> inspection, Enter the number "801" for a PRE-COVER inspec- <br /> tion. You may leave a message for the inspector if desired. <br /> Be sure the System Record form with the As-Built drawing is <br /> on the job or in our office when the inspection is requested <br /> An inspection should be completed within seven (7) days. <br /> <br /> <br />
