SITE SPECIFIC HEALTH AND SAFETY PLAN
This health and safety plan has been developed for field activities to be performed at
petroleum hydrocarbon release sites and other hazardous material cleanup sites. Field
personnel may be exposed to or involved with mechanical excavation and removal of
underground storage tanks, contaminated soil and fill materials (soils), backfilling of solid
materials, drilling activities, loud noises, harmful airborne contaminants, or other health and
safety factors.
The purpose of this health and safety program is to provide guidelines and standard operating
procedures necessary to ensure the health and safety of on-site workers, the general public,
and the environment. Implementation of this program will be the responsibility of the Project
Manager. Because the field work is subject to on-site conditions (i.e. weather, unforeseen
field conditions, etc.) the plan may need to be modified to accommodate unplanned events.
~ The components of this health and safety plan include: pre-entry briefing; site hazards
evaluation; personal protective clothing and equipment requirements; field monitoring for
~ airborne contaminants; a site layout plan including decontamination procedures; emergency
plan; and Plan Review and Daily Health and Safety Log.
~ 1.1 Pre-entry Briefing
The pre-entry briefing is to bring to workers awareness how the site safetly plan is to be
~ followed. The briefing is held at the start of each day prior to initiating any site activity.
~ The information is to be completed by the Health and Safety Officer (Project Manager) on a
~ daily basis prior to the start of work activities.
GENERAL INFORMATION
~ Site Name
Address
~ Euaene, Oreqon
Telephone Contact Tien Nquven Eu4ene School District 4J,
(541)687-3259
~ i I Mumford
Pro~ect Manager Dan e
Health and Safety Officer Daniel Mumford
~ Date December 14-22, 1995
` Time 0700-1700
2
