INSTRUCTIONS FOR COMPLETING PAYROLL/CERTIFIED STATEMENT FORM, <br />WH-38 (Rev 6/96) <br />Column 8- Total Deductions. FICA. FED. STATE. ETC: Enter the total amount of deductions withheid from <br />each employee for just those hours reported on this payroll/certified statement for this project. All deductions <br />must be in accordance with the provisions of ORS 652.610. <br />Column 9- Net Wages Paid for Week; Enter the amount of wage actually paid to the employee after <br />subtracting the total deductions reported in Column 8 from the gross amount earned shown in Column 7. <br />Column 10 - Hourly Fringe Benefit Paid to Party. Plan. Fund or Program: Enter the hourly amount of fringe <br />benefits paid to each individually approved party, plan, fund or program for each employee. List these amounts <br />separately on the lines provided. Any contactor who is making payments to approved parties, plans, funds or <br />programs in amounts less than the reauired hourly fringe benefit is obligated to pay the difference directly to <br />the employee as wages in lieu of fringe benefits, and to show that amount in Column 6 of this form. <br />Column 1 1- Name of Benefit Party. Plan. Fund or Program: Enter the name of the party, plan, fund or program <br />that corresponds to the amount shown as an hourty fringe benefit in Column 10. <br />Summarv - In order to determine if the wages and fringe benefits being certified by this statement are sufficient <br />to meet prevailing wage rate requirements, the following check may be performed: <br />1. Consider each Trade Cpassification listed in Column 2. <br />2. For that Trade Classification, take the sum of: <br />a) the Basic Hourly Rate of Pay (Column 5-, <br />b) the Hourly Fringe Benefit Paid as Wage to Employee (Column 6), <br />c) and the Hourly Fringe Benefit Paid To Party, Plan, Fund or Program (Column 101. <br />3. This sum must equal or exceed the sum of the Basic Hourly Rate (including zone pay and special <br />wage differentials, if any) and the Fringe Benefit as they are listed for that Trade Classification <br />in the appropriately dated issue of the Bureau of Labor and Industries publication Prevailing Waae <br />Rates for Public Works Contracts in Oregon. <br />, ~ <br />NOTICE TO CONTRACTORS: YOU ARE NO LONGER REQUIRED TO SUBMIT COPIES OF THE PAYROLL/CERTIFIED <br />STATEMENT TO THE BUREAU OF LABOR AND INDUSTRIES.
