Leonard Lodder, AIA <br />April 26, 1999 <br />Page 2 <br />The City may bill the permit holder for costs of inspection that exceed the permit fee. This will apply <br />to exceptional situations such as, but not limited to, repeated failure of testing, repeated removal and <br />replacement of defective work, wasted inspection trips for work that was not ready to inspect, trips <br />to meet or inspect when the contractor is a no-show, etc. <br />Should it be determined that exceptional costs will be recovered, a billing will be sent to the permit <br />holder. The project will not be given a final inspection nor will an amended improvement agreement <br />be issued until the costs are paid. <br />Notes for traffic control: Every contractor working in the City right-of-way shall be required to <br />obtain a Street/Lane/Alley/Sidewalk Closure Permit from the Public Works Department Traffic <br />Engineering Section, Room 325, two working days in advance and provide signing diagrams for all <br />closures. Required closure fees must be paid before permits are issued. Each occurrence of working <br />in the right-of-way will require a separate permit. Every contractor working in the right-of-way <br />outside the city of Salem shall contact the appropriate City, County, or State jurisdiction for right-of- <br />way permits. <br />If the construction permit has not been issued within six months of the date of this letter, the <br />approval is null and void in accordance with Salem Revised Code 77.091(c), unless a time extension <br />is requested. It is the developer's responsibility to contact the City for a time extension. The City <br />will not notify the developer when the six months plan approval expires. The fees for this <br />construction permit are based on the estimate of the cost of construction at the time of the original <br />approval letter. Fees are revised effective July 1 of each year. If a time extension is granted, fees will <br />be recalculated based on the fee schedule in effect at the time the extension is granted. <br />Prior to issuance of the construction permit, we must receive evidence of contractors' insurance and <br />worker's compensation coverage, and a 100 percent performance security made for the contract <br />amount. The criteria for acceptable form of performance security may be obtained by calling the <br />Construction Inspection Section at (503) 588-621 l. <br />Prior to issuance of the construction permit, the contractor shall submit a construction schedule. A <br />formal, detailed construction schedule in accordance with Section 108.01 of the City's Standard <br />Construction Specifications, indicating the sequence of construction for the principal items of work, <br />will be required.
