The unit pre-treats the septic tank effluent before it is discharged into the ground. The owner of a Sand Filter,RGF or ATT system must maintain a service contract with a licensed and certified professional to service and maintain the onsite system. A service contract must be entered into before the system is installed and must be maintained until the system is decommissioned. Specific Requirements Please refer to the Site Evaluation Worksheet which shows the approved areas and other details of the site visit. A CURTAIN DRAIN(GROUNDWATER INTERCEPTOR)IS REQUIRED as part of this system to divert groundwater from the drainfield. The curtain drain trench must be constructed a minimum12 inches wide by 42 inches deep with 30 inches of clean gravel placed over a 4 inch perforated pipe(ASTM Specification F 405, D 2729 or F 810). Gravel must be placed a minimum of 2 inches under the pipe. The gravel must be covered with filter fabric. Outlet pipe must be rigid,smooth-wall, solid PVC pipe meeting or exceeding ASTM Standard D-3034. The outlet must be fitted with a flap gate or rodent guard. The curtain drain must be located at least 10 feet up slope from the disposal field and must discharge through a watertight non-perforated pipe beside or below the disposal field. A minimum separation of 10 feet is required from the septic tank if the tank is located up slope of the curtain drain and 5 feet if the tank is located down slope of the curtain drain. Additional Conditions of Site Approval 1. This site is approved for the type of disposal system described above. Peak sewage flow into the system is limited to a maximum of 450 gallons per day,with an average sewage flow of not more than 225 gallons per day. This is normally sufficient to serve a single family dwelling with a maximum of four bedrooms. Premature failure of the treatment system may occur if either of these flow quantities is exceeded. If,for some reason,you expect your domestic household water use may exceed these flows,it may be advisable to increase the size of the treatment system. 2. Any alteration of natural soil conditions(i.e.cutting or filling)in the acceptable area may void this approval. 3. Both the initial and replacement disposal areas are to be protected from traffic, livestock and farming activities, development or other potential disturbance of natural soil conditions. 4. The area must not be subjected to excessive saturation due to,but not limited to,artificial drainage of ground surfaces, roads,driveways and building down spouts. 5. This approval is given on the basis that the parcel described above will not be further partitioned or subdivided. 6. Placement of a well within 100 feet of the approved areas may invalidate this approval. THIS IS NOT YOUR PERMIT.A Construction/Installation permit is required before you construct your system. Please contact this office when you are ready to apply for a construction/installation permit.We cannot sign off on any Building Codes forms until we issue your permit. This site approval runs with the land and will automatically benefit subsequent owners.This site approval is valid until the approved system is constructed under a DEQ construction permit or unless the site is altered without approval from this office. Alterations/excavations/lot line adjustments made to the site, or placement of wells or utilities,etc.,may invalidate this approval If you disagree with the decision of this report,you may apply for a site evaluation report review. The application for a site evaluation report review must be submitted to DEQ in writing within 60 days after the site evaluation report issue date and must include the site evaluation review fee in OAR 340-071-0140 Table 9A. A senior DEQ staff person will be assigned the site evaluation report review application. You may apply for a variance to the onsite wastewater treatment system rules. The variance application must include a copy of the site evaluation report,plans and specifications for the proposed system,specify the rule(s)to which a variance is being requested,demonstrate the variance is warranted, and include the variance fee in OAR 340-071-140 Table 9C. A variance may only be granted if the variance officer determines that strict compliance with a rule is inappropriate or special physical conditions render strict compliance unreasonable, burdensome or impractical. A senior DEQ variance officer will be assigned the variance application.
