• <br /> Building Inspection Division <br /> N�u . arson Count <br /> Onsite Septic Review <br /> 0RE.GON <br /> PUBLIC WORKS <br /> Activity Number: 555-18-008880-INQY Date: November 29,2018 <br /> Project Address: 5995 Golden Hill Lane SE, Salem <br /> Parcel 1 of 2,Partition Case 18-023; 2 acres of TL 082W16C 01900 <br /> Completed By: Matthew Puntney, REHS <br /> RECORD REVIEW <br /> This existing system review was conducted in response to application for Partition Case No 18-023. The <br /> existing dwelling is located on a 5.5 acre lot that will be adjusted to 2 acres. <br /> Installation Permit Record: Marion County has records that indicate the existing system was installed in <br /> 1976 under permit number 188. It consists of a 1000 gallon tank and 450 linear feet of serpentine drainline. <br /> Components and Function: The Existing System Evaluation was completed by A&B Septic Service on <br /> July 5,2017. The technician noted a 1000 gallon tank and no signs of failure. <br /> A site visit was conducted on November 29, 2018. The system appeared to be in use and showed no signs of <br /> failure. The distribution box was exposed and appeared to be functioning normally. <br /> A test pit was evaluated to establish an area for a repair system. The area around the pit is suitable for a <br /> STANDARD SYSTEM. Please refer to the field worksheet for further instruction detail. <br /> The septic system area should be protected from any vehicular traffic, livestock and farming activities. Other <br /> activities such as logging, excavating or filling, driveway placement or any similar activities may damage the <br /> soil in the area of the drainfield and adversely affect system function. A RISER TO THE GROUND <br /> SURFACE IS STRONGLY RECOMMENDED TO FACILITATE EASIER PUMPING. <br /> The septic tank should be pumped and inspected by a licensed septic tank pumping service when sludge <br /> and scum accumulation reaches 30% of tank volume. Pumping every 3-5 years is an average interval. <br /> The septic tank is a reservoir for the solid materials in the sewage waste stream. Excessive <br /> accumulation of these solid materials in the septic tank will cause the solids to wash out of the tank and <br /> into the drainfield. This will cause irreversible damage to the drainfield and will eventually lead to a <br /> system failure. <br /> Disclaimer: This evaluation report describes the on-site system as it exists on the date of inspection and to <br /> the extent that components and operation of the system are reasonably observable.
