The proposed installation plan is approved as revised. Please refer to the approved revised
site plans dated July 24,2017. Any deviation from the approved plans must be authorized by this office prior to installation or
the permit may be voided. It's the responsibility of applicant/installer to coordinate these requirements with all others
in OAR 340-071-0220.
This Permit is for a system designed to serve a single family dwelling with a maximum of 5 bedrooms. Peak sewage flow into
the system is limited to a maximum of 525 gallons per day, with an average sewage flow of-not more than 263 gallons per
day. Premature failure of the treatment system may occur if either of these flow quantities is exceeded. If, for some reason,
you expect your domestic household water use may exceed these flows, it is advisable to increase the size of the treatment
system.
EFFLUENT SEWER:The effluent sewer is the pipe from the septic tank to the distribution box. The material required is 3034
or Schedule 40 pipe with 18-gauge minimum green tracer wire or green metallic tape placed along the pipe. The effluent
sewer shall extend at least five(5)feet from the tank. There must be a minimum difference of eight(8)inches between the
invert(flow line)of the septic tank outlet and the invert of the distribution or drop box outlet. In Equal Distribution, there must
be six(6)inches of fall between the septic tank outlet and the distribution box inlet. In Serial Distribution, there must be two(2)
inches of fall between the septic tank outlet and the drop box inlet.
A CURTAIN DRAIN (GROUNDWATER INTERCEPTOR)IS REQUIRED as part of this system to divert groundwater from the
drainfield. The curtain drain trench must be constructed a minimum 12 inches wide by 42 inches deep with 30 inches of clean
gravel placed over a 4 inch perforated pipe(ASTM Specification F 405, D 2729 or F 810). Gravel must be placed a minimum
of 2 inches under the pipe. The gravel must be covered with filter fabric. Outlet pipe must be rigid,smooth-wall,solid PVC
pipe meeting or exceeding ASTM Standard D-3034. The outlet must be fitted with a flap gate or rodent guard. The curtain
drain must be located at least 10 feet up slope from the disposal field and must discharge through a watertight non-perforated
pipe beside or below the disposal field. A minimum separation of 10 feet is required from the septic tank if the tank is located
up slope of the curtain drain and 5 feet if the tank is located down slope of the curtain drain.
DRY SOIL CONDITIONS ARE REQUIRED FOR INSTALLATION(March 1—October 31).The geographical area in which
the property lies is considered a risk to installation during winter months. Soil moisture conditions generally preclude
construction of the drainfield in the winter months.
OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. A copy of the DEQ setback
requirements is on file in our office and available online at http://www.deq.state.or.us/wq/onsite/sdssearch.asp under Rules:
Division 71:Table 1.
THE APPROVED SEPTIC SYSTEM AREA MUST BE.PROTECTED from any vehicular traffic, livestock and farming
activities. Other activities such as excavating or filling, driveway placement, paving or other similar activities that alter the
natural soil conditions will adversely affect the soil and the septic system function and lead to premature system failure. The
area must not be subjected to excessive saturation due to artificial drainage of ground surfaces, driveways, building
downspouts or other sources. The areas of the initial septic system and the future repair system must both be protected.
CALL BEFORE YOU DIG...IT'S THE LAW
ATTENTION:Oregon law requires you to follow rules adopted by the Oregon Utility Notification Center. Those rules are set forth by Oregon Administration Rules.
