<br /> This property has been approved for a STANDARD SEPTIC SYSTEM in the area identified in Site Evaluation Report 555-18-000511-EVAL. System
<br /> plans reflect the approved stakeout and are approved as submitted. Refer to approved plans dated February 26,2018. Any deviation from the
<br /> approved plans must be authorized by this office prior to installation or the permit may be voided. It's the responsibility of applicant/installer to coordinate
<br /> these requirements with all others in OAR 340-071-0220.
<br /> • This permit is for a system designed to serve a single family dwelling with a maximum of 4 bedrooms.Peak sewage flow into the system is limited to a
<br /> maximum of 450 gallons per day,with an average sewage flow of not more than 225 gallons per day. Premature failure of the treatment system may
<br /> occur if either of these flow quantities is exceeded. If,for some reason,you expect your domestic household water use may exceed these flows,it is
<br /> advisable to increase the size of the treatment system.
<br /> PRESSURE OPERATED EQUAL DISTRIBUTION IS REQUIRED as part of this installation. TRENCH DEPTHS are 24"minimum/24"maximum and
<br /> require a pressure-operated hydrosplitter to achieve equal distribution. Lines leading from hydrosplitter to distribution boxes must drain freely,with no
<br /> sag.
<br /> AN EFFLUENT LIFT PUMP WILL BE REQUIRED to install the septic system disposal field in the approved area and at the required depth. Detailed
<br /> instructions for dosing tank/effluent pump installations are available in our office. The electrical wiring to the pump and alarm must be installed in
<br /> accordance with the appropriate Electrical Code. The electrical work must be done under a Marion County Electrical Permit and must be inspected and
<br /> approved by a Marion County Electrical Inspector. The work must be left uncovered and accessible for inspection. The electrical inspection includes the
<br /> connections to the pump and alarm in the dosing tank,the wiring in the trench and the connections in the structure. The effluent pipe must have
<br /> 18-gauge minimum green tracer wire or green metallic tape placed along the line.
<br /> A CURTAIN DRAIN(GROUNDWATER INTERCEPTOR)IS REQUIRED as part of this system to divert groundwater from the drainfield. The curtain
<br /> drain trench must be constructed a minimuml2 inches wide by 42 inches deep with 30 inches of clean gravel placed over a 4 inch perforated pipe
<br /> (ASTM Specification F 405,D 2729 or F 810). Gravel must be placed a minimum of 2 inches under the pipe. The gravel must be covered with filter
<br /> fabric. Outlet pipe must be rigid,smooth-wall,solid PVC pipe meeting or exceeding ASTM Standard D-3034. The outlet must be fitted with a flap gate
<br /> or rodent guard. The curtain drain must be located at least 10 feet up slope from the disposal field and must discharge through a watertight
<br /> non-perforated pipe beside or below the disposal field. A minimum separation of 10 feet is required from the septic tank if the tank is located up slope of
<br /> the curtain drain and 5 feet if the tank is located down slope of the curtain drain.
<br /> • HEADER PIPE between Distribution Box and drainfield trench must measure a minimum of 4 feet,must be installed level,and bedded on undisturbed
<br /> earth.
<br /> • TANK RISER: The tank must be installed with watertight risers extending to the ground surface or above. The riser lid must be gasketed and
<br /> securely fastened.
<br /> •DRY SOIL CONDITIONS ARE REQUIRED FOR INSTALLATION(March 1—October 31) The geographical area in which the property lies is
<br /> considered a risk to installation during winter months. Soil moisture conditions generally preclude construction of the drainfield in the winter months.
<br /> • OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. A copy of the DEQ setback requirements is on file in
<br /> our office and available online at http://www.oregon.gov/deq/Residential/Pages/Onsite.aspx under Rules:Division 71:Table 1.
<br /> MAINTAIN PERMANENT ACCESS to septic tank for pumping and service.The septic tank should be pumped and inspected by a licensed septic
<br /> tank pumping service before sludge and scum accumulations reach 30%of the tank liquid operating volume. Pumping every 3 to 5 years is an average
<br /> interval. The septic tank is a reservoir for the solid materials in the sewage waste stream. Excessive accumulation of these solid materials in the septic
<br /> tank will cause the solids to wash out of the tank and into the drainfield. This will cause irreversible damage to the drainfield and will eventually lead to a
<br /> system failure.
<br /> • THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic,livestock and farming activities. Other activities such
<br /> as excavating or filling,driveway placement,paving or other similar activities that alter the natural soil conditions will adversely affect the soil and the
<br /> septic system function and lead to premature system failure. The area must not be subjected to excessive saturation due to artificial drainage of ground
<br /> surfaces,driveways,building downspouts or other sources. The areas of the initial septic system and the future repair system must both be protected.
<br /> Installation of this onsite wastewater treatment system has been determined to comply with the applicable requirements in
<br /> Oregon Administrative Rules Chapter 340, Divisions 071 and 073 and the Conditions of Approval above.
<br /> 1. In accordance with Oregon Revised Statute 454,665,this Certificate of Satisfactory Completion is issued as evidence of
<br /> satisfactory completion of an onsite wastewater treatment system at the location identified above.
<br /> 2. Issuance of this Certificate does not constitute a warranty or guarantee that this onsite wastewater treatment system will
<br /> function indefinitely without failure. Conditions imposed as permit requirements continue for the life of the system.
<br /> 3.The area of the initial and the identified replacement area must not be subjected to activity that is likely to adversely
<br /> affect the soil or the functioning of the system. Such activities may include,but are not limited to,vehicular traffic, livestock,
<br /> covering the area with asphalt or concrete,filling,cutting, or other soil modification activities.
<br /> 4.This onsite wastewater treatment system that be connected to the facility referenced herein within 5 years of the
<br /> issuance of this Certificate of Satisfactory Completion(CSC)or rules for authorization notices,alteration permits, or
<br /> construction-installation permits as outlined in OAR 340-071-0160,340-071-0205, or 340-071-0210 apply, including
<br /> payment of an additional fee.
<br /> 5.This system must operate in compliance with OAR Chapter 340, Division 071 and must not create a public health hazard
<br /> or pollute public waters.
<br /> 6. Unless otherwise required by the agent,the system installer must backfill(cover)this system within 10 days after the
<br /> issuance of this Certificate of Satisfactory Completion.
