Septic Permit 555-18-005721-PRMT Page 2 of 3 <br /> Date•Certificate Issued:03/26/2019 <br /> Work Description:, NEW INSTALL: 4 BDRM SFD <br /> Conditions=of Approval <br /> Approved Area&System Type: <br /> This property has been approved for a STANDARD SEPTIC SYSTEM in the area identified in Site Evaluation Report <br /> 555-16-003232. System plans reflect the approved stakeout and are approved as submitted. Refer to approved plans dated <br /> August 27,2018. Any deviation from the approved plans must be authorized by this office prior to installation or the permit may <br /> be voided. It's the responsibility of applicant/installer to coordinate these requirements with all others in OAR 340-071-0220. <br /> This permit is for a system designed to serve a single family dwelling with a maximum of 4 bedrooms.Peak sewage flow into the <br /> system is limited to a maximum of 450 gallons per day,with an average sewage flow of not more than 225 gallons per day. <br /> Premature failure of the treatment system may occur if either of these flow quantities is exceeded. If,for some reason,you <br /> expect your domestic household water use may exceed these flows, it is advisable to increase the size of the treatment system. <br /> Specific Requirements: <br /> EFFLUENT SEWER:The effluent sewer is the pipe from the septic tank to the distribution box. The material required is 3034 or <br /> Schedule 40 pipe with 18-gauge minimum green tracer wire or green metallic tape placed along the pipe. The effluent sewer <br /> shall extend at least five(5)feet from the tank. There must be a minimum difference of eight(8)inches between the invert(flow <br /> line)of the septic tank outlet and the invert of the drop box outlet. In Serial Distribution there must be two(2)inches of fall <br /> between the septic tank outlet and the drop box inlet. <br /> HEADER PIPE between Distribution Box and drainfield trench must measure a minimum of 4 feet,must be installed level,and <br /> bedded on undisturbed earth. <br /> TANK RISER: The tank must be installed with watertight risers extending to the ground surface or above. The riser lid must be <br /> gasketed and securely fastened. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. A copy of the DEQ setback <br /> requirements is available in our office and online at http://www.oregon.gov/deq/Residential/Pages/Onsite-Resources.aspx and <br /> scroll to Table 1,"Minimum Separation Distances." <br /> DO NOT INSTALL SYSTEM DURING WINTER MONTHS(Nov.1—March 1) <br /> The geographical area in which the property lies is considered a risk to installation during winter months. Therefore,for this <br /> installation a Marion County Onsite Wastewater Specialist must specifically approve soil moisture conditions at this site for <br /> installation to occur during the period from November 1st to March 1st. This may include a field visit directly prior to installation. <br /> MAINTAIN PERMANENT ACCESS to septic tank for pumping and service.The septic tank should be pumped and <br /> inspected by a licensed septic tank pumping service before sludge and scum accumulations reach 30%of the tank <br /> liquid operating volume. Pumping every 3 to 5 years is an average interval. The septic tank is a reservoir for the solid <br /> materials in the sewage waste stream. Excessive accumulation of these solid materials in the septic tank will cause the solids to <br /> wash out of the tank and into the drainfield. This will cause irreversible damage to the drainfield and will eventually lead to a <br /> system failure. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTEDfrom any vehicular traffic,livestock and farming activities. <br /> Other activities such as excavating or filling,driveway placement,paving or other similar activities that alter the natural soil <br /> conditions will adversely affect the soil and the septic system function and lead to premature system failure. The area must not <br /> be subjected to excessive saturation due to artificial drainage of ground surfaces,driveways,building downspouts or other <br /> sources.
