<br /> The septic tank installation plan is approved as submitted. Please refer to the signed approved site plan dated July 16,2018.
<br /> Any deviation from the approved plans must be authorized by this office prior to installation or the permit may be voided.
<br /> TANK RISER: The tank must be installed with watertight risers extending to the ground surface or above. The riser lid must be
<br /> gasketed and securely fastened.
<br /> EFFLUENT SEWER:The effluent sewer is the pipe from the septic tank to the distribution box. The material required is 3034 or
<br /> Schedule 40 pipe with 18-gauge minimum green tracer wire or green metallic tape placed along the pipe. The effluent sewer
<br /> shall extend at least five(5)feet from the tank. There must be a minimum difference of eight(8)inches between the invert(flow
<br /> line)of the septic tank outlet and the invert of the distribution or drop box outlet. In Equal Distribution,there must be six(6)
<br /> inches of fall between the septic tank outlet and the distribution box inlet. In Serial Distribution,there must be two(2)inches of
<br /> fall between the septic tank outlet and the drop box inlet.
<br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the Department of Environmental Quality rules. A
<br /> copy of the DEQ setback requirements is available in our office and online at
<br /> http://www.oregon.gov/deq/Residential/Pages/Onsite.aspx under Rules: Division 71:Table 1.
<br /> THE ATTACHED SEPTIC TANK ABANDONMENT FORM MUST BE COMPLETED AND RETURNED TO OUR OFFICE. The
<br /> septic system installer must complete and sign the form and attach a copy of the receipt from the septic tank pumping service
<br /> for pumping the tank.
<br /> MAINTAIN PERMANENT ACCESS to septic tank for pumping and service. The septic tank should be pumped and
<br /> inspected by a licensed septic tank service before sludge and scum accumulations reach 30%of the tank liquid
<br /> operating volume. Pumping every 3 to 5 years is an average interval. The septic tank is a reservoir for the solid materials
<br /> in the sewage waste stream. Excessive accumulation of these solid materials in the septic tank will cause the solids to wash out
<br /> of the tank and into the drainfield. This will cause irreversible damage to the drainfield and will eventually lead to a system
<br /> failure.
<br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTEDfrom any vehicular traffic or other sources of soil
<br /> compaction. Activities such as excavating or filling,driveway placement, paving or any similar alteration of the natural soil
<br /> conditions will adversely affect septic system function and lead to premature system failure. The area must not be subjected to
<br /> excessive saturation due to artificial drainage of ground surfaces,driveways,building downspouts or other sources. The areas
<br /> of the initial septic system and the future repair system must both be protected.
<br /> Installation of this onsite wastewater treatment system has been determined to comply with the applicable requirements in
<br /> Oregon Administrative Rules Chapter 340, Divisions 071 and 073 and the Conditions of Approval above.
<br /> 1. In accordance with Oregon Revised Statute 454,665,this Certificate of Satisfactory Completion is issued as evidence of
<br /> satisfactory completion of an onsite wastewater treatment system at the location identified above.
<br /> 2. Issuance of this Certificate does not constitute a warranty or guarantee that this onsite wastewater treatment system will
<br /> function indefinitely without failure. Conditions imposed as permit requirements continue for the life of the system.
<br /> 3.The area of the initial and the identified replacement area must not be subjected to activity that is likely to adversely
<br /> affect the soil or the functioning of the system.Such activities may include, but are not limited to,vehicular traffic, livestock,
<br /> covering the area with asphalt or concrete,filling, cutting, or other soil modification activities.
<br /> 4.This onsite wastewater treatment system that be connected to the facility referenced herein within 5 years of the
<br /> issuance of this Certificate of Satisfactory Completion(CSC)or rules for authorization notices, alteration permits, or
<br /> construction-installation permits as outlined in OAR 340-071-0160, 340-071-0205, or 340-071-0210 apply, including
<br /> payment of an additional fee.
<br /> 5.This system must operate in compliance with OAR Chapter 340, Division 071 and must not create a public health hazard
<br /> or pollute public waters.
<br /> 6. Unless otherwise required by the agent,the system installer must backfill(cover)this system within 10 days after the
<br /> issuance of this Certificate of Satisfactory Completion.
