Please refer to the signed approved site plan dated March 4,2019. Any deviation from the approved plans must be authorized by this office prior to installation or the permit may be voided. This permit is issued in conjunction with an application for a hardship residence at the address 3139 Jefferson-Scio Drive SE. The Authorization cannot be completed until the tank is installed and a Certificate of Satisfactory Completion is issued. TANK: 1000 gallon poly tank as per submitted plans. TANK RISER: The tank must be installed with watertight risers extending to the ground surface or above. The riser lid must be gasketed and securely fastened. The soil in the area appears suitable for a poly or plastic tank. EFFLUENT SEWER:The effluent sewer is the pipe from the septic tank to the distribution box. The material required is 3034 or Schedule 40 pipe with 18-gauge minimum green tracer wire or green metallic tape placed along the pipe. The effluent sewer shall extend at least five(5)feet from the tank. There must be a minimum difference of eight(8)inches between the invert(flow line)of the septic tank outlet and the invert of the distribution or drop box outlet. In Equal Distribution,there must be six(6) inches of fall between the septic tank outlet and the distribution box inlet. In Serial Distribution,there must be two(2)inches of fall between the septic tank outlet and the drop box inlet. OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the Department of Environmental Quality rules. MAINTAIN PERMANENT ACCESS to septic tank for pumping and service. The septic tank should be pumped and inspected by a licensed septic tank service before sludge and scum accumulations reach 30%of the tank liquid operating volume. Pumping every 3 to 5 years is an average interval. The septic tank is a reservoir for the solid materials in the sewage waste stream. Excessive accumulation of these solid materials in the septic tank will cause the solids to wash out of the tank and into the drainfield. This will cause irreversible damage to the drainfield and will eventually lead to a system failure. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic or other sources of soil compaction. Activities such as excavating or filling,driveway placement, paving or any similar alteration of the natural soil conditions will adversely affect septic system function and lead to premature system failure. The area must not be subjected to excessive saturation due to artificial drainage of ground surfaces,driveways,building downspouts or other sources. The areas of the initial septic system and the future repair system must both be protected Installation of this onsite wastewater treatment system has been determined to comply with the applicable requirements in Oregon Administrative Rules Chapter 340, Divisions 071 and 073 and the Conditions of Approval above. 1. In accordance with Oregon Revised Statute 454,665,this Certificate of Satisfactory Completion is issued as evidence of satisfactory completion of an onsite wastewater treatment system at the location identified above. 2. Issuance of this Certificate does not constitute a warranty or guarantee that this onsite wastewater treatment system will function indefinitely without failure. Conditions imposed as permit requirements continue for the life of the system. 3.The area of the initial and the identified replacement area must not be subjected to activity that is likely to adversely affect the soil or the functioning of the system. Such activities may include, but are not limited to,vehicular traffic, livestock, covering the area with asphalt or concrete,filling,cutting, or other soil modification activities. 4.This onsite wastewater treatment system that be connected to the facility referenced herein within 5 years of the issuance of this Certificate of Satisfactory Completion(CSC)or rules for authorization notices, alteration permits, or construction-installation permits as outlined in OAR 340-071-0160, 340-071-0205, or 340-071-0210 apply, including payment of an additional fee. 5.This system must operate in compliance with OAR Chapter 340, Division 071 and must not create a public health hazard or pollute public waters. 6.
