The conditions observed during this site <br /> evaluation indicate that the area around the evaluated test holes is CAPABLE of supporting a STANDARD SEPTIC SYSTEM. <br /> Please refer to the signed approved site plan dated March 16,2017. Any deviation from the approved plans must be authorized <br /> by this office prior to installation or the permit may be voided. Refer to field worksheet for additional details. <br /> EFFLUENT SEWER:The effluent sewer is the pipe running from the septic tank to the disposal field. The effluent sewer shall <br /> extend at least five(5)feet from the tank. There must be a minimum difference of eight(8)inches between the invert(flow line) <br /> of the septic tank outlet and the invert of the distribution or drop box outlet. In Equal Distribution,there must be six(6)inches of <br /> fall between the septic tank outlet and the distribution box inlet. In Serial Distribution,there must be two(2)inches of fall <br /> between the septic tank outlet and the drop box inlet. <br /> DRY SOIL CONDITIONS ARE REQUIRED FOR INSTALLATION(March 1—October 31)The geographical area in which the <br /> property lies is considered a risk to installation during winter months. Soil moisture conditions generally preclude construction of <br /> the drainfield in the winter months.THE ATTACHED SEPTIC TANK ABANDONMENT FORMMUST BE COMPLETED AND <br /> RETURNED TO OUR OFFICE. The septic system installer must complete and sign the form and attach a copy of the receipt <br /> from the septic tank pumping service for pumping the tank. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. A copy of the DEQ setback <br /> requirements is on file in our office and available online at http://www.deq.state.or.us/wq/onsite/sdssearch.aspunder Rules: <br /> Division 71:Table 1. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities. <br /> Other activities such as excavating or filling,driveway placement,paving or other similar activities that alter the natural soil <br /> conditions will adversely affect the soil and the septic system function and lead to premature system failure. The area must not <br /> be subjected to excessive saturation due to artificial drainage of ground surfaces,driveways, building downspouts or other <br /> sources. The areas of the initial septic system and the future repair system must both be protected. <br /> THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL. When work is complete and ready for <br /> inspection,request an inspection by following the instructions on the accompanying permit document. Be sure the System <br /> Record form with the As-Built drawing is on the job or in our office when the inspection is requested. An inspection should be <br /> completed within seven(7)days. <br /> Installation of this onsite wastewater treatment system has been determined to comply with the applicable requirements in <br /> Oregon Administrative Rules Chapter 340, Divisions 071 and 073 and the Conditions of Approval above. <br /> 1. In accordance with Oregon Revised Statute 454,665,this Certificate of Satisfactory Completion is issued as evidence of <br /> satisfactory completion of an onsite wastewater treatment system at the location identified above. <br /> 2. Issuance of this Certificate does not constitute a warranty or guarantee that this onsite wastewater treatment system will <br /> function indefinitely without failure. Conditions imposed as permit requirements continue for the life of the system. <br /> 3.The area of the initial and the identified replacement area must not be subjected to activity that is likely to adversely <br /> affect the soil or the functioning of the system. Such activities may include, but are not limited to,vehicular traffic, livestock, <br /> covering the area with asphalt or concrete,filling, cutting,or other soil modification activities. <br /> 4.This onsite wastewater treatment system that be connected to the facility referenced herein within 5 years of the <br /> issuance of this Certificate of Satisfactory Completion(CSC)or rules for authorization notices, alteration permits, or <br /> construction-installation permits as outlined in OAR 340-071-0160,340-071-0205, or 340-071-0210 apply, including <br /> payment of an additional fee. <br /> 5.This system must operate in compliance with OAR Chapter 340, Division 071 and must not create a public health hazard <br /> or pollute public waters. <br /> 6.
