The conditions observed during this site evaluation indicate that the area around the evaluated test holes is CAPABLE of supporting a STANDARD SEPTIC SYSTEM. Please refer to the signed approved site plan dated March 16,2017. Any deviation from the approved plans must be authorized by this office prior to installation or the permit may be voided. Refer to field worksheet for additional details. EFFLUENT SEWER:The effluent sewer is the pipe running from the septic tank to the disposal field. The effluent sewer shall extend at least five(5)feet from the tank. There must be a minimum difference of eight(8)inches between the invert(flow line) of the septic tank outlet and the invert of the distribution or drop box outlet. In Equal Distribution,there must be six(6)inches of fall between the septic tank outlet and the distribution box inlet. In Serial Distribution,there must be two(2)inches of fall between the septic tank outlet and the drop box inlet. DRY SOIL CONDITIONS ARE REQUIRED FOR INSTALLATION(March 1—October 31)The geographical area in which the property lies is considered a risk to installation during winter months. Soil moisture conditions generally preclude construction of the drainfield in the winter months.THE ATTACHED SEPTIC TANK ABANDONMENT FORMMUST BE COMPLETED AND RETURNED TO OUR OFFICE. The septic system installer must complete and sign the form and attach a copy of the receipt from the septic tank pumping service for pumping the tank. OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. A copy of the DEQ setback requirements is on file in our office and available online at http://www.deq.state.or.us/wq/onsite/sdssearch.aspunder Rules: Division 71:Table 1. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities. Other activities such as excavating or filling,driveway placement,paving or other similar activities that alter the natural soil conditions will adversely affect the soil and the septic system function and lead to premature system failure. The area must not be subjected to excessive saturation due to artificial drainage of ground surfaces,driveways, building downspouts or other sources. The areas of the initial septic system and the future repair system must both be protected. THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL. When work is complete and ready for inspection,request an inspection by following the instructions on the accompanying permit document. Be sure the System Record form with the As-Built drawing is on the job or in our office when the inspection is requested. An inspection should be completed within seven(7)days. Installation of this onsite wastewater treatment system has been determined to comply with the applicable requirements in Oregon Administrative Rules Chapter 340, Divisions 071 and 073 and the Conditions of Approval above. 1. In accordance with Oregon Revised Statute 454,665,this Certificate of Satisfactory Completion is issued as evidence of satisfactory completion of an onsite wastewater treatment system at the location identified above. 2. Issuance of this Certificate does not constitute a warranty or guarantee that this onsite wastewater treatment system will function indefinitely without failure. Conditions imposed as permit requirements continue for the life of the system. 3.The area of the initial and the identified replacement area must not be subjected to activity that is likely to adversely affect the soil or the functioning of the system. Such activities may include, but are not limited to,vehicular traffic, livestock, covering the area with asphalt or concrete,filling, cutting,or other soil modification activities. 4.This onsite wastewater treatment system that be connected to the facility referenced herein within 5 years of the issuance of this Certificate of Satisfactory Completion(CSC)or rules for authorization notices, alteration permits, or construction-installation permits as outlined in OAR 340-071-0160,340-071-0205, or 340-071-0210 apply, including payment of an additional fee. 5.This system must operate in compliance with OAR Chapter 340, Division 071 and must not create a public health hazard or pollute public waters. 6.
