The conditions observed <br /> during this site evaluation indicate that the area around the evaluated test holes is CAPABLE of supporting a <br /> STANDARD SEPTIC SYSTEM. Please refer to the signed approved site plan dated March 16,2017.Any <br /> deviation from the approved plans must be authorized by this office prior to installation or the permit may be <br /> voided. Refer to field worksheet for additional details. <br /> EFFLUENT SEWER:The effluent sewer is the pipe running from the septic tank to the disposal field. The- <br /> effluent sewer shall extend at least five (5)feet from the tank. There must be a minimum difference of eight <br /> (8) inches between the invert(flow line) of the septic tank outlet and the invert of the distribution or drop box <br /> outlet. In Equal Distribution,there must be six (6) inches of fall between the septic tank outlet and the <br /> distribution box inlet. In Serial Distribution, there must be two (2) inches of fall between the septic tank <br /> outlet and the drop box inlet. <br /> DRY SOIL CONDITIONS ARE REQUIRED FOR INSTALLATION (March 1 —October 31)The geographical <br /> area in which the property lies is considered a risk to installation during winter months. Soil moisture <br /> conditions generally preclude construction of the drainfield in the winter months.THE ATTACHED SEPTIC <br /> TANK ABANDONMENT FORMMUST BE COMPLETED AND RETURNED TO OUR OFFICE. The septic system <br /> installer must complete and sign the form and attach a copy of the receipt from the septic tank pumping <br /> service for pumping the tank. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. A copy of the <br /> DEQ setback requirements is on file in our office and available online at <br /> http://www.deq.state.or.us/wq/onsite/sdssearch.aspunder Rules: Division 71: Table 1. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and <br /> farming activities. Other activities such as excavating or filling, driveway placement, paving or other similar <br /> activities that alter the natural soil conditions will adversely affect the soil and the septic system function and <br /> lead to premature system failure. The area must not be subjected to excessive saturation due to artificial <br /> drainage of ground surfaces,driveways, building downspouts or other sources. The areas of the initial septic <br /> system and the future repair system must both be protected. <br /> THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL. When work is complete and <br /> ready for inspection, request an inspection by following the instructions on the accompanying permit <br /> document. Be sure the System Record form with the As-Built drawing is on the job or in our office when the <br /> inspection is requested.
