Please refer to the signed approved site plan dated June 26,2018. Any deviation from the approved plans must be authorized by this office prior to installation or the permit may be voided. TANK RISER: The tank must be installed with watertight risers extending to the ground surface or above. The riser lid must be gasketed and securely fastened. EFFLUENT SEWER:The effluent sewer is the pipe from the septic tank to the distribution box. The material required is 3034 or Schedule 40 pipe with 18-gauge minimum green tracer wire or green metallic tape placed along the pipe. The effluent sewer shall extend at least five(5)feet from the tank. There must be a minimum difference of eight(8)inches between the invert(flow line)of the septic tank outlet and the invert of the distribution or drop box outlet. In Equal Distribution,there must be six(6)inches of fall between the septic tank outlet and the distribution box inlet. In Serial Distribution,there must be two(2)inches of fall between the septic tank outlet and the drop box inlet. OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the Department of Environmental Quality rules. THE ATTACHED SEPTIC TANK ABANDONMENT FORM MUST BE COMPLETED AND RETURNED TO OUR OFFICE. The septic system installer must complete and sign the form and attach a copy of the receipt from the septic tank pumping service for pumping the tank. MAINTAIN PERMANENT ACCESS to septic tank for pumping and service. The septic tank should be pumped and inspected by a licensed septic tank service before sludge and scum accumulations reach 30%of the tank liquid operating volume. Pumping every 3 to 5 years is an average interval. The septic tank is a reservoir for the solid materials in the sewage waste stream. Excessive accumulation of these solid materials in the septic tank will cause the solids to wash out of the tank and into the drainfield. This will cause irreversible damage to the drainfield and will eventually lead to a system failure. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTEDfrom any vehicular traffic or other sources of soil compaction. Activities such as excavating or filling,driveway placement, paving or any similar alteration - of the natural soil conditions will adversely affect septic system function and lead to premature system failure. The area must not be subjected to excessive saturation due to artificial drainage of ground surfaces,driveways, building downspouts or other sources.
