<br /> This property has been approved for a STANDARD SEPTIC SYSTEM in the area identified in Site Evaluation Report 98-09222.
<br /> System plans are approved as submitted. Refer to approved plans dated May 8,2018. Any deviation from the approved plans
<br /> must be authorized by this office prior to installation or the permit may be voided. It's the responsibility of applicant/installer to
<br /> coordinate these requirements with all others in OAR 340-071-0220.This permit is for a system designed to serve a single
<br /> family dwelling with a maximum of 4 bedrooms. Peak sewage flow into the system is limited to a maximum of 450 gallons per
<br /> day,with an average sewage flow of not more than 225 gallons per day. Premature failure of the treatment system may occur if
<br /> either of these flow quantities is exceeded. If,for some reason,you expect your domestic household water use may exceed
<br /> these flows, it is advisable to increase the size of the treatment system. EFFLUENT SEWER:The effluent sewer is the pipe from
<br /> the septic tank to the distribution box. The material required is 3034 or Schedule 40 pipe with 18-gauge minimum green tracer
<br /> wire or green metallic tape placed along the pipe. The effluent sewer shall extend at least five(5)feet from the tank. There
<br /> must be a minimum difference of eight(8)inches between the invert(flow line)of the septic tank outlet and the invert of the
<br /> distribution or drop box outlet. In Serial Distribution,there must be two(2)inches of fall between the septic tank outlet and the
<br /> drop box inlet. HEADER PIPE between Distribution Box and drainfield trench must measure a minimum of 4 feet, must be
<br /> installed level, and bedded on undisturbed earth. TANK RISER: The tank must be installed with watertight risers extending to
<br /> the ground surface or above. The riser lid must be gasketed and securely fastened. OBSERVE ALL SETBACKS AND
<br /> SEPARATION DISTANCES as required by the DEQ rules. A copy of the DEQ setback requirements is on file in our office and
<br /> available online at http://www.deq.state.or.us/wq/onsite/sdssearch.asp under Rules: Division 71:Table 1. MAINTAIN
<br /> PERMANENT ACCESS to septic tank for pumping and service.The septic tank should be pumped and inspected by a licensed
<br /> septic tank pumping service before sludge and scum accumulations reach 30%of the tank liquid operating volume. Pumping
<br /> every 3 to 5 years is an average interval. The septic tank is a reservoir for the solid materials in the sewage waste stream.
<br /> Excessive accumulation of these solid materials in the septic tank will cause the solids to wash out of the tank and into the
<br /> drainfield. This will cause irreversible damage to the drainfield and will eventually lead to a system failure.
<br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities.
<br /> Other activities such as excavating or filling,driveway placement, paving or other similar activities that alter the natural soil
<br /> conditions will adversely affect the soil and the septic system function and lead to premature system failure. The area must not
<br /> be subjected to excessive saturation due to artificial drainage of ground surfaces, driveways, building downspouts or other
<br /> sources. The areas of the initial septic system and the future repair system must both be protected.
<br /> Installation of this onsite wastewater treatment system has been determined to comply with the applicable requirements in
<br /> Oregon Administrative Rules Chapter 340, Divisions 071 and 073 and the Conditions of Approval above.
<br /> 1. In accordance with Oregon Revised Statute 454,665,this Certificate of Satisfactory Completion is issued as evidence of
<br /> satisfactory completion of an onsite wastewater treatment system at the location identified above.
<br /> 2. Issuance of this Certificate does not constitute a warranty or guarantee that this onsite wastewater treatment system will
<br /> function indefinitely without failure. Conditions imposed as permit requirements continue for the life of the system.
<br /> 3.The area of the initial and the identified replacement area must not be subjected to activity that is likely to adversely
<br /> affect the soil or the functioning of the system. Such activities may include, but are not limited to,vehicular traffic, livestock,
<br /> covering the area with asphalt or concrete,filling, cutting,or other soil modification activities.
<br /> 4.This onsite wastewater treatment system that be connected to the facility referenced herein within 5 years of the
<br /> issuance of this Certificate of Satisfactory Completion(CSC)or rules for authorization notices,alteration permits, or
<br /> construction-installation permits as outlined in OAR 340-071-0160,340-071-0205, or 340-071-0210 apply, including
<br /> payment of an additional fee.
<br /> 5.This system must operate in compliance with OAR Chapter 340, Division 071 and must not create a public health hazard
<br /> or pollute public waters.
<br /> 6. Unless otherwise required by the agent,the system installer must backfill(cover)this system within 10 days after the
<br /> issuance of this Certificate of Satisfactory Completion.
