'-',.... :7,-, <br /> Approved Area&System Type <br /> This property has been approved for an ALTERNATIVE TREATMENT TECHNOLOGY Treatment Standard 1 in the area <br /> identified in Site Evaluation Report 555-15-002092. System plans are approved as submitted. Refer to approved plans dated <br /> September 6,2018. The installation must conform to these approved plans. Any changes or product substitutions must be <br /> approved by this office prior to installation. Any substantial changes may require complete revised plans with a plan review fee. <br /> Failure to comply may invalidate construction approval and permit. The required inspection schedule is attached to this permit. <br /> It's the responsibility of applicant/installer to coordinate these requirements with all others in OAR 340-071-0220. <br /> This approval is for a system designed to serve a single family dwelling with a maximum of 4 bedrooms. Peak sewage flow into <br /> the system is limited to a maximum of 450 gallons per day,with an average sewage flow of not more than 225 gallons per day. <br /> Premature failure of the treatment system may occur if either of these flow quantities is exceeded. If,for some reason,you <br /> expect your domestic household water use may exceed these flows, it is advisable to increase the size of the treatment system. <br /> Specific Requirements <br /> ATT Pretreatment Unit: Advantex AX20 RT as per approved attached plan. <br /> SERVICE CONTRACTS: The owner of an AU system must maintain a service contract with a maintenance provider certified <br /> by the manufacturer to serve and maintain the onsite system. A service contract must be entered into before the system is <br /> installed and must be maintained until the system is decommissioned. The service contract must provide for a minimum of four <br /> inspections and service visits by a maintenance provider scheduled once every six months over a two-year period to inspect, <br /> adjust, and service the AU. The maintenance provider must submit an annual report and fee. Please review OAR <br /> 340-071-0345 for full requirements. <br /> TANK RISER: The tank must be installed with watertight risers extending to the ground surface or above. The riser lid must be <br /> gasketed and securely fastened. <br /> ELECTRICAL COMPONENTS ARE REQUIRED FOR INSTALLATION OF THIS SEPTIC SYSTEM. Detailed instructions for <br /> dosing tank/effluent pump installations are available in our office. The electrical wiring to the pump and alarm must be installed <br /> in accordance with the appropriate Electrical Code. The electrical work must be done under a Marion County Electrical Permit <br /> and must be inspected and approved by a Marion County Electrical Inspector. The work must be left uncovered and accessible <br /> for inspection. The electrical inspection includes the connections to the pump and alarm in the dosing tank,the wiring in the <br /> trench and the connections in the structure. <br /> HEADER PIPE must measure a minimum of 4 feet between Distribution Box and start of drainfield trench,must be installed <br /> level, and bedded on undisturbed earth. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. A copy of the DEQ setback <br /> requirements is on file in our office and available online at http://www.deq.state.or.us/wq/onsite/sdssearch.asp under Rules: <br /> Division 71:Table 1. <br /> DO NOT INSTALL SYSTEM DURING WINTER MONTHS(Nov.1—March 1) <br /> The geographical area in which the property lies is considered a risk to installation during winter months. Therefore,for this <br /> installation a Marion County Onsite Wastewater Specialist must specifically approve soil moisture conditions at this site for <br /> installation to occur during the period from November 1st to March 1st. This may include a field visit directly prior to installation. <br /> MAINTAIN PERMANENT ACCESS to septic tank for pumping and service. The septic tank must be pumped before solids <br /> reach 30%of the tank liquid operating level. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular traffic, livestock and farming activities. <br /> Other activities such as excavating or filling,driveway placement, paving or other similar activities that alter the natural soil <br /> conditions will adversely affect the soil and the septic system function and lead to premature system failure. The area must not <br /> be subjected to excessive saturation due to artificial drainage of ground surfaces,driveways, building downspouts or other <br /> sources.
