Septic Permit 555-18-004377-PRMT Page 2 of 3 <br /> Date Certificate Issued 06/03/2019 <br /> Work Description NEW INST 3 BDRM SF.D P17 016 <br /> �� •-... <br /> Conditions of Approval <br /> This property has been approved for a STANDARD SEPTIC SYSTEM in the area identified in Site Evaluation Report <br /> 555-18-004377. System plans are approved as submitted. Refer to approved ed tans dated June 28,2018. Any deviation from <br /> the approved plans must be authorized by this office prior to installation or the permit may be voided. Its the responsibility of <br /> applicant/installer to coordinate these requirements with all others in OAR 340-071-0220. <br /> This permit is for a system designed to serve a single family dwelling with a maximum of 4 bedrooms.Peak sewage flow into the <br /> system is limited to a maximum of 450 gallons per day,with an average sewage flow of not more than 225 gallons per day. <br /> Premature failure of the treatment system may occur if either of these flow quantities is exceeded. If,for some reason,you <br /> expect your domestic household water use may exceed these flows, it is advisable to increase the size of the treatment system. <br /> EFFLUENT SEWER:The effluent sewer is the pipe from the septic tank to the distribution box. The material required is 3034 or <br /> Schedule 40 pipe with 18-gauge minimum green tracer wire or green metallic tape placed along the pipe. The effluent sewer <br /> shall extend at least five(5)feet from the tank. There must be a minimum difference of eight(8)inches between the invert(flow <br /> line)of the septic tank outlet and the invert of the distribution or drop box outlet. <br /> HEADER PIPE between Distribution Box and drainfield trench must measure a minimum of 4 feet,must be installed level,and <br /> bedded on undisturbed earth. <br /> TANK RISER: The tank must be installed with watertight risers extending to the ground surface or above. The riser lid must be <br /> gasketed and securely fastened. <br /> OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. A copy of the DEQ setback <br /> requirements is on file in our office and available online at http://www.deq.state.or.us/wq/onsite/sdssearch.asp under Rules: <br /> Division 71:Table 1. <br /> MAINTAIN PERMANENT ACCESS to septic tank for pumping and service.The septic tank should be pumped and inspected by <br /> a licensed septic tank pumping service before sludge and scum accumulations reach 30%of the tank liquid operating volume. <br /> Pumping every 3 to 5 years is an average interval. The septic tank is a reservoir for the solid materials in the sewage waste <br /> stream. Excessive accumulation of these solid materials in the septic tank will cause the solids to wash out of the tank and into <br /> the drainfield. This will cause irreversible damage to the drainfield and will eventually lead to a system failure. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTEDfrom any vehicular traffic,livestock and farming activities. <br /> Other activities such as excavating or filling,driveway placement,paving or other similar activities that alter the natural soil <br /> conditions will adversely affect the soil and the septic system function and lead to premature system failure. The area must not <br /> be subjected to excessive saturation due to artificial drainage of ground surfaces,driveways,building downspouts or other <br /> sources. The areas of the initial septic system and the future repair system must both be protected. <br /> Installation of this onsite wastewater treatment system has been determined to comply with the applicable requirements in <br /> Oregon Administrative Rules Chapter 340, Divisions 071 and 073 and the Conditions of Approval above. <br /> 1. In accordance with Oregon Revised Statute 454,665,this Certificate of Satisfactory Completion is issued as evidence of <br /> satisfactory completion of an onsite wastewater treatment system at the location identified above. <br /> 2. Issuance of this Certificate does not constitute a warranty or guarantee that this onsite wastewater treatment system will <br /> function indefinitely without failure.Conditions imposed as permit requirements continue for the life of the system. <br /> 3.The area of the initial and the identified replacement area must not be subjected to activity that is likely to adversely <br /> affect the soil or the functioning of the system.Such activities may include,but are not limited to,vehicular traffic,livestock, <br /> covering the area with asphalt or concrete,filling,cutting,or other soil modification activities. <br /> 4.This onsite wastewater treatment system that be connected to the facility referenced herein within 5 years of the <br /> issuance of this Certificate of Satisfactory Completion(CSC)or rules for authorization notices,alteration permits,or <br /> construction-installation permits as outlined in OAR 340-071-0160,340-071-0205,or 340-071-0210 apply, including <br /> payment of an additional fee. <br /> 5.This system must operate in compliance with OAR Chapter 340, Division 071 and must not create a public health hazard <br /> or pollute public waters. <br /> 6.Unless otherwise required by the agent,the system installer must backfill(cover)this system within 10 days after the <br /> issuance of this Certificate of Satisfactory Completion. <br /> 6/3/19:3:11:45PM Page 2 of 3 ONS_OnsiteCSC_pr <br />
