Conditions of Approval This property has been approved for a STANDARD SEPTIC SYSTEM in the area identified in Site Evaluation Report 555-18-004377. System plans are approved as submitted. Refer to approved ed tans dated June 28,2018. Any deviation from the approved plans must be authorized by this office prior to installation or the permit may be voided. Its the responsibility of applicant/installer to coordinate these requirements with all others in OAR 340-071-0220. This permit is for a system designed to serve a single family dwelling with a maximum of 4 bedrooms.Peak sewage flow into the system is limited to a maximum of 450 gallons per day,with an average sewage flow of not more than 225 gallons per day. Premature failure of the treatment system may occur if either of these flow quantities is exceeded. If,for some reason,you expect your domestic household water use may exceed these flows, it is advisable to increase the size of the treatment system. EFFLUENT SEWER:The effluent sewer is the pipe from the septic tank to the distribution box. The material required is 3034 or Schedule 40 pipe with 18-gauge minimum green tracer wire or green metallic tape placed along the pipe. The effluent sewer shall extend at least five(5)feet from the tank. There must be a minimum difference of eight(8)inches between the invert(flow line)of the septic tank outlet and the invert of the distribution or drop box outlet. HEADER PIPE between Distribution Box and drainfield trench must measure a minimum of 4 feet,must be installed level,and bedded on undisturbed earth. TANK RISER: The tank must be installed with watertight risers extending to the ground surface or above. The riser lid must be gasketed and securely fastened. OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. A copy of the DEQ setback requirements is on file in our office and available online at http://www.deq.state.or.us/wq/onsite/sdssearch.asp under Rules: Division 71:Table 1. MAINTAIN PERMANENT ACCESS to septic tank for pumping and service.The septic tank should be pumped and inspected by a licensed septic tank pumping service before sludge and scum accumulations reach 30%of the tank liquid operating volume. Pumping every 3 to 5 years is an average interval. The septic tank is a reservoir for the solid materials in the sewage waste stream. Excessive accumulation of these solid materials in the septic tank will cause the solids to wash out of the tank and into the drainfield. This will cause irreversible damage to the drainfield and will eventually lead to a system failure. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTEDfrom any vehicular traffic,livestock and farming activities. Other activities such as excavating or filling,driveway placement,paving or other similar activities that alter the natural soil conditions will adversely affect the soil and the septic system function and lead to premature system failure. The area must not be subjected to excessive saturation due to artificial drainage of ground surfaces,driveways,building downspouts or other sources. The areas of the initial septic system and the future repair system must both be protected. Installation of this onsite wastewater treatment system has been determined to comply with the applicable requirements in Oregon Administrative Rules Chapter 340, Divisions 071 and 073 and the Conditions of Approval above. 1. In accordance with Oregon Revised Statute 454,665,this Certificate of Satisfactory Completion is issued as evidence of satisfactory completion of an onsite wastewater treatment system at the location identified above. 2. Issuance of this Certificate does not constitute a warranty or guarantee that this onsite wastewater treatment system will function indefinitely without failure.Conditions imposed as permit requirements continue for the life of the system. 3.The area of the initial and the identified replacement area must not be subjected to activity that is likely to adversely affect the soil or the functioning of the system.Such activities may include,but are not limited to,vehicular traffic,livestock, covering the area with asphalt or concrete,filling,cutting,or other soil modification activities. 4.This onsite wastewater treatment system that be connected to the facility referenced herein within 5 years of the issuance of this Certificate of Satisfactory Completion(CSC)or rules for authorization notices,alteration permits,or construction-installation permits as outlined in OAR 340-071-0160,340-071-0205,or 340-071-0210 apply, including payment of an additional fee. 5.This system must operate in compliance with OAR Chapter 340, Division 071 and must not create a public health hazard or pollute public waters. 6.Unless otherwise required by the agent,the system installer must backfill(cover)this system within 10 days after the issuance of this Certificate of Satisfactory Completion. 6/3/19:3:11:45PM Page 2 of 3 ONS_OnsiteCSC_pr
