555-19-004012-AUTH <br /> 7070 Liberty Road S, Salem <br /> 083W28A 00800 <br /> June 14,2019 Completed By: Matthew Puntney, REHS <br /> ONSITE WASTEWATER TREATMENT SYSTEM AUTHORIZATION NOTICE <br /> Existing system information: <br /> Marion County <br /> records show the existing septic system serving this property was <br /> installed in 1978 under permit number 541. It consisted of a 1000 gallon tank and <br /> 300 linear feet of serpentine drainline. The distribution method was altered to a <br /> serial system to correct a failing line in 2012 under permit number 12-00764. Drop <br /> boxes were added. A Certificate of Satisfactory Completion is on file for both <br /> permits. <br /> The system was last pumped and inspected by A&B Septic on April 22, 2019. The <br /> pumper noted a 1000 gallon tank and no sign of failure. <br /> A field visit was made in response to an application to connect a 3-bedroom dwelling <br /> to the existing system. The system was in use at the time of the visit. No signs of <br /> failure were present. There is adequate room for a repair system. The proposed <br /> dwelling will meet all setbacks to the system. <br /> Flow increase information: <br /> The site plan shows the proposed structures and concurrent site development can <br /> maintain all required setbacks to the existing system. Placement of a three-bedroom <br /> single family dwelling will not exceed the design flow of this system. This meets the <br /> criteria listed in OAR 340-071-0205(4) (a)-(c) and is an allowable connection to this <br /> system. THIS AUTHORIZATION TO CONNECT TO THE EXISTING ONSITE SYSTEM IS <br /> APPROVED. Please refer to the signed approved site plan dated June 14, 2019. <br /> THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any vehicular <br /> traffic, livestock and farming activities. Other activities such as logging, excavating <br /> or filling, driveway placement, paving or any similar activities may also damage the <br /> soil in the area and adversely affect the septic system approval. The areas of the <br /> initial septic system and the future repair system must both be protected. <br /> THE SEPTIC TANK SHOULD BE PUMPED AND INSPECTED by a licensed septic tank <br /> pumping service approximately EVERY FIVE (5) YEARS. The septic tank should be <br /> pumped before solids reach 30% of the tank liquid operating level. The septic tank <br /> is a reservoir for the solid materials in the sewage waste stream. Excessive <br /> accumulation of these solid materials in the septic tank will cause the solids to wash <br /> out of the tank and into the drainfield.
