St. Paul <br /> DIRECTOR <br /> Brian Nicholas, P.E. <br /> Dear Mark, <br /> ADMINISTRATION <br /> On December 02, 2015, septic installation permit 555-15-007824 was <br /> BUILDING issued for the installation of a repair septic system located at the above <br /> INSPECTION mentioned address to serve one single family dwelling and a business that <br /> serves 6 employees An inspection was done on 11/25/16 and 07/20/17, by <br /> EMERGENCY Matt Puntney and the septic system installation was approved. However, a <br /> MANAGEMENT signed asbuilt document with a materials list and map of the approved <br /> ENGINEERING installation was never submitted to this office. An asbuilt document signed <br /> by the licensed and certified septic installer that has installed the system or <br /> ENVIRONMENTAL the homeowner that has installed the septic system is required before a <br /> SERVICES Certificate of Satisfactory Completion can be issued by this office. <br /> OPERATIONS Per OAR 340-071-0175(6), A person may not connect to or use any <br /> PARKS system completed after January 1, 1974, unless a Certificate of <br /> Satisfactory Completion has been issued for the installation or deemed <br /> PLANNING issued by operation of law in accordance with this rule. <br /> SURVEY Per OAR 340-071-160(5), Permit effective dates, A permit issued for <br /> construction of a system under this rule is effective for one year from the <br /> date of issuance.
