Completed By: Jessica L.Joye, REHS, Onsite Wastewater Specialist ONSITE WASTEWATER TREATMENT SYSTEM INSTALLATION Approved Area & System Type This property has been approved for a STANDARD SEPTIC SYSTEM in the area of the approved test holes. Please refer to the signed approved site plan dated November 8, 2012. Any deviation from the approved plans must be authorized by this office prior to installation or the permit may be voided Specific Requirements EFFLUENT SEWER:The effluent sewer is the pipe from the septic tank to the distribution or drop box. The material required is 3034 or Schedule 40 pipe with 18-gauge minimum green tracer wire or green metallic tape placed along the pipe. The effluent sewer shall extend at least 5-feet from the tank. There must be a minimum difference of 8-inches between the invert(flow line)of the septic tank outlet and the invert of the distribution or drop box outlet. In Equal Distribution, there must be 6-inches of fall between the septic tank outlet and the distribution box inlet, and 2-inches of drop within the box. In Serial Distribution, there must be 2-inches of fall between the septic tank outlet and the drop box inlet, and 6-inches of drop within the box. AN EFFLUENT LIFT PUMP WILL BE REQUIRED to install the septic system disposal field in the approved area and at the required depth.Detailed instructions for dosing tank/effluent pump installations are available in our office. The electrical wiring to the pump and alarm must be installed in accordance with the appropriate Electrical Code. The electrical work must be done under a Marion County Electrical Permit and must be inspected and approved by a Marion County Electrical Inspector.The work must be left uncovered and accessible for inspection. The electrical inspection includes the connections to the pump and alarm in the dosing tank, the wiring in the trench and the connections in the structure. OBSERVE ALL SETBACKS AND SEPARATION DISTANCES as required by the DEQ rules. A copy of the DEQ setback requirements is included for your review. Additional Conditions of Site Approval 1. This site is approved for the type of disposal system described above. Peak sewage flow into the system is limited to a maximum of 625 gallons per day, with an average sewage flow of not more than 313 gallons per day. Approval is limited to 1-ry space and 5 employees. Premature failure of the treatment system may occur if either of these flow quantities is exceeded.
