Form 1.7.2A Revised Au.ust 2021
North Side-
The structure's north side had no smoke, heat, or fire damage to the structure's exterior.
I checked in with command and was told the structure was safe to enter and that some crews were still working on
overhauling the west bedroom. At this point, I began my interior examination.
INTERIOR EXAMINATION—
I entered the structure through the front door located on the east side of the structure. This led me into the front living
room and entry area. In this area, there did not appear to be much smoke or heat damage, however, there was
extensive damage from the fire screws performing overhaul and extinguishment activities.
I then entered the dining room and kitchen area. This area too, appeared to have no smoke or heat damage but had
extensive damage from overhaul and extinguishment activities.
I then proceed down the hallway walking towards the west. On the north side of the hallway, there was a bedroom. This
bedroom had light smoke damage but appeared not to have any heat damage. Like the other areas, this room had
extensive overhaul and extinguishment damage On the south side of the hallway was a restroom. This restroom had
light smoke damage and light heat damage from the fire. There was extensive damage from overhaul and
extinguishment activities and a moderate amount of fire debris from the attic space.
Continuing down the hallway to the west, I entered the master bedroom. Heavy damage to the ceiling and roof was
noted on entry, and was watched closely for any hazards. Additionally, the floor sustained heavy damage from
underneath due to the fire in the crawl space. This, too, was monitored closely for any hazards. On the south wall, there
were two walk-in closets. These closets had smoke damage but little to no heat damage.
Facing the west in the master bedroom, there appeared to be the remains of a sliding glass door leading to the patio.
Above that, there was extensive damage to the ceiling and attic area. Crews were working to extinguish any hot spots
when I was photographing. I had them briefly clear the area, obtained my photos, and had them resume their work.
To the northwest corner of the bedroom, there was the master bathroom. This bathroom had extensive damage from
overhaul and extinguishment activities and a moderate amount of fire debris from the attic space.
At this point, I confirmed that the fire had started on the structures exterior. Therefore, I moved to the west patio area
for a follow-up examination.
EXTERIOR FOLLOW-UP EXAMINATION-
Engine nine and sevens crew and I went to the structure's west side, where the patio was. We began by slowly layering
the wood decking. At this point, I noted that the wood had heavier charring on the bottom(IMG_617.JPG)and
appeared the be charred to nearly total thickness. This led me to believe that the fire began under the patio and burned
for an extended period before it became fully involved.
Moving from north to south, the boards appeared to have been burned even more. Around the center of the patio, in
front of the sliding glass door, the boards were completely burned away in approximately a 4 ft2 x 4 ft2 area
(IMG_0620.JPG).
Wth the burned area and the account from Mr. Norman about smoking on the back patio, this appears to have been
the area of origin.
AREA OF ORIGIN—
Investigator: Date: Incident Number.
Page 5 of
Justin Guinan,DFM IAAI-FIT July 11th,2023 2023-00015078
