A service contract must be entered into <br /> before the system is installed and must be maintained until the system is decommissioned. <br /> This repair site approval is for a system designed to serve a single-family dwelling with a <br /> maximum of three bedrooms. Peak sewage flow into the system is limited to a maximum of 375 <br /> gallons per day, with an average sewage flow of not more than 225 gallons per day. Premature <br /> failure of the treatment system may occur if either of these flow quantities is exceeded. If you <br /> expect your domestic household water use may exceed these flows, it is advisable to increase <br /> the size of the treatment system. <br /> Submittals required for permit issuance: <br /> 1. A plot plan showing a full system stakeout of the proposed repair. This must also include <br /> the actual location of all existing drainline(s) and setbacks to the escarpment, property <br /> lines, and proposed home. A guide to stakeout requirements is attached to this letter. <br /> 2.
