However, the site appearc suitable for
the installation of an alternative seepage trenchec as deccribed in the
attached construction detail and setback sheets.

This approval is given on the basis that the lot or parcel will not be
further partitioned or subdivided and that conditions on this property and on
adjacent properties ale not altered in any manner which would prohibit the
issuance of a permit. Soils in the approved area shall not be altered or
modified by cutting, filling, or other developmental activities which would
impede the suCCessful operation of the initial or replacement soil absorption
syctems. Failure to comply with the above items may void thic approval.

~hiB report is not a per~it to i~stall the sewaq9' system. A permit must
~e obtained before any system con~tructio~ occurs. A permit may be issued only
after the property has been found to be compatible with applicable LCDC
acknowledged local comprehensive land use plan. A Statement o~ Land Use
Compatibility or its equivalent is required before a construction permit can
be issued.

A detailed plot plan, drawn to scale (ex. 1" ~ 60'), must be submitted
with the permit application before the construction-installation permit can be
issued. The system must be installed by the property owner or a D~Q licensed
septic installer.

This site evaluation report is valid until an on-site sewage system is
installed pursuant to the construction permit obtained from this office, or
until earlier cancellation pursuant to the Environmental Quality Co~mission
~ules. Subject to the above, this repor~ runs with the land and will automati-
sally benefit subsequent owners.

Our office hours are Monday through Friday, 8:00 to 4:30.
