Th~ Septic system is D~o~sed to serve a three <br />bedroom residence on the property. <br /> <br /> Based on the ~esult~ of this study, this area does not appear suitabl~ <br />for a standard on-site sewage system. However, the sit~ appears sui~abl~ for <br />the installation of an alternative SAND FILTER PLUS FULL CAPPING FILL as <br />described in the attached construction de=ail and setback sheets. <br /> <br /> This approval is given on the basis that the lot or parcel will not be <br />further parti=iened or subdivided and that conditions on this property and on <br />adjacent properties are not altered in any mauner which wo~ld prohibit the <br />issuance of a per, it. soils in ~he approved area shall not be altered or <br />~odified by ~utting, filling, or other developmental activities which would <br />impede %h~ ~ucceesful e~e~ation of the initial or r~placem~nt soil absorption <br />systems. Failure to comply with the above items may void this approval. <br /> <br /> Thi~. repert [~...ngt a ~ermit tg....install the sew~i~ system. A De~it must <br />be obtained befor~ a~¥ svstem con~tructiDn occurs. A permit may be issued only <br />after the property has been found to be compatible wi:h applicable LCDC <br />acknowledged local comprehensive land ~e plan. A Statement of Land Use <br />Compatibility or its ec/~ivalent is r~quirsd before a construction permit can <br />be issued. <br /> <br /> a d~tailed plot plan, drawn to ~¢ale (ex. 1" ~ 60'), must be submitted <br />with the ~ermit application before the construction-installation permit can be <br />issued. The system mu~ b~ installed by the property owner or a DEQ licensed <br />septic installer. <br /> <br /> Thi~ ~ite evaluation report is valid until an on-site sewage system is <br />installed pursuant =o the construction permit obtained from this office, or <br />until earlier cancellation pursuant to the Environmental Quality Commission <br />rules.
