AUMSVILL~, ORr. 97352 <br /> <br /> Our office has compl,te~ the re-evaluation for the proposed £nstallation <br />of an on-site sewage syste~. The septic syate~ is proposed to serve a three <br />bedroom on the property. <br /> <br /> Based on the re~lts Of this study, on-~ite ~ewage dis~sal appear~ <br />feasible for the ~roperty. However, site conditions will r~quire t~e installa- <br />tion Of a PRESSURIZED DISTRIBUTION SYSTE~ for the original system and a SAND <br />FILTER for the future re~lac~ent system. These are described in the attached <br />construction de, ail and setback sh~ts. <br /> <br /> This approval is given on ~he basis that the lot o~ parcel will not be <br />further partitioned oN subdivided and that condi~ion~ on this property and on <br />adjacent properties are not altered in any manner which would prohibit the <br />issuance o~ a permit. $oi1~ in the a~proved area shall not be altere~ or <br />~odified by cut%lng, ~illin9, o~ other developmental activities which would <br />£~pede the successful operation of the initial or repiacement soil absorption <br />systems. Failure to Comply with the above items may void this approval. <br /> <br /> ~his..reDort is not a permit to install the sewaqp, system~ A Der~it must <br />~p obtained ~efore an~ ~¥stemconstrection o~eurs. A per, it may b~ issued only <br />after %he p~operty has been found te be compatible with applicable LCDC <br />acknowledged local cOmprehensive land U~e plan. A Statement of Land Use <br />compatibility or its equivalent is required before a construction permit can <br />be issued. <br /> <br /> A detailed plot plan, d~awn =o seals (ex. 1" ~ 60'), must be submitted <br />with the permit application b~fore the construction-installation permit can be <br />issued. The system ~u~t be installed by the property owner or a DEQ licensed <br />septic i~stall~r. <br /> <br /> This site evaluation ~epof~ is valid until an on-site sewage system is <br />in,tailed pursuant ~o ~he constructio~ permit obtaine~ from this office, or <br />until earlier cancellation pursuan~ to the Environmental Quality Con%~ission <br />rules.
