Based on the results of this study, site evaluation <br />approval is ~yanted. The approvai is site specific to the area tested, and the septic system <br />must be located in the immediate area of the test pits, unlessOtherwise authorized by this <br />office. A new site evaluation may be required if the system is proposed for a different <br />area on the property. <br /> <br />This approval .... is ~iven on the basis that the lot or ~arcel will not be further partitioned' * <br />or subdivided and that conditions on this property and on adjacent properties are not altered in <br />any manner which would prohibit the issuance of a permit. Soils in the approved area shall not~b <br />altered or modified by cuttinge filling, or other developmental activities which would impede the <br />successful operation of the initial or replacement soil absorption systems. Failure to comply <br />the above items may void this approval. <br /> <br />This report is not a permit to install the sewage system. A permit must be obtained before <br />any system construction occurs. A permit may be issued only after the property has been found to <br />be compatible with applicable LCDC acknowledged local comprehensive land use plan. A Statement o~ <br />Land Use Compatibility or its equivalent is required before a construction permit can be issued. <br /> <br />A d~tailed site plan. drawn to scale (ex: i" = 6B"), must be submitted with the permit <br />application before the construction-installation permit can be issued. The system must be <br />installed by the property owner or a registered contractor~ <br /> <br />This site evaluation report is valid until an on-site sewage system is installed pursuamt to the <br />construction permit obtained from this office, or until cancelation pursuant to the Environmental <br />Quality Commission Rules. SOIL CONDITIONS AT THE TEST PITS ARE MARGINAL TO~N- <br />SIDER STANDARD SYSTEM DRAINFIELDS PROTECTED WITH AN UP-SLOPE <br />CURTAIN DRIAN. DRAINLINES MUST BE INSTALLED WITHIN THE AREA <br />OF THE TEST PITS. USE ~ARE NOT TO ALTER THE NATURAL TOPSOIL <br />~ITHIN THE DRAINFIELDS SITES PRIOR TO INSTALLATION.
