The approval is site specific to the area tested, and the septic system <br />must be located in the immediate area of the test pits, unless otherwise authorized by this <br />office. A new site evaluation may be required if the system is proposed for a different <br />area on the property. <br /> <br />This approval is given on the basis that the lot or parcel will not be further partitioned <br />or subdivided and that conditions on this property and on adjacent properties are not altered in <br />any manner which would prohibit the issuance of a permit. Soils in the ~pproved ares shall not be <br />altered or mbdified by cutting, filling, or other developmental activities which would impede the <br />successful operation of the initial or replacement soil absorption systems, Failure to comply with <br />t~e above items may void this approval, <br /> <br />This report is not a permit to install the sewage system, A permit must be obtained before <br />any system construction occurs. A permit may be issued only after the property has been found to <br />be compatible with applicable LCDC acknowledged local comprehensive land use plan. A Statement of <br />Land Use Compatibility or its equivalent is required before a construction permit can be issued, <br /> <br />A detailed site plan. drawn to scale (ex: 1" = 60"), must be submitted with the permit <br />application before the construction-installation permit can be issued. The system must be <br />installed by the property owner or a registered contractor. <br /> <br />This site evaluation report is valid until an on-site sewage system is installed pursuant to the <br />construction permit obtained from this office, or until cancelation pursuant to the Environmental <br />Quality Commission Rules. Subject to the above, this report runs with the land and will a~tom~t- <br />ically benefit subsequent owners. <br /> <br />Site conditions will require the following installation specifications: <br /> <br />TYPE OF INITIAL SYSTEM: SAND FILTER <br /> <br />MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS <br />LINEAL FEET OF DRAINFIELD: 135 <br />TYPE OF DISTRIBUTION: EQUAL <br />DISPOSAL TRENCH DEPTH: 12INCHES. MINIMUM: 12INCHES, MAXIMUM <br />TOTAL DRAIN MEDIA DEPTH: 12 <br />DRAIN MEDIA UNDER PIPE: 6 <br />MINIMUM SOIL BACKFILL: 10 <br />GROUND WATER INTERCEPTOR: N <br /> <br /> TYPE OF FUTURE REPLACEMENT SYSTEM: SAND FILTER <br /> <br />Before any installation takes place, a permit must first be obtained from Marion County. <br /> <br />COMMENTS AND CONDITIONS <br /> <br />1. THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON MAY 15. <br /> 1998. SOIL CONDITIONS AT THE TEST PITS APPEAR MARGINALLY <br /> SUITABLE FOR SAND FILTER PLUS FULL CAPPING FILL DRAINFIELDS. <br /> DRAINFIELDS MUST BE LOCATED ON THE HIGHEST LANDFORM WITHIN <br /> THE IMMEDIATE AREA OF THE TEST PITS. THE TEST PIT SITES MUST <br /> BE STAKED FOR FUTURE LOCATION IF BACKFLLED FOR SAFETY, NOTE, <br /> THE DRY SOIL, SUMMER SEASON CONSTRUCTION REQUIREMENTS FOR <br /> THE CAPPING FILL DRAINFIELDS. USE CARE NOT TO ALTER THE TOP- <br /> SOIL WITHIN THE DRAINFIELD SITES PRDR TO INSTALLATION, THE <br /> SAND FILTER MAY NEED TO BE ELEVATED FOR GRAVITY FLOW INTO <br /> THE SHALLOW DRAINFIELD LINES, <br />2. THE A~ACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection, The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br />2.
