A new site evaluation may be required if the system is proposed for a different <br />area on the property. <br /> <br />This approval is g~ven on the basis that the lot or parcel will not be further partitioned <br />or subdivided and that conditions on th~s property and on adjacent properties are not altered ~n <br />any manner which would prohibit the ~ssuance of a permit. Soils in the approved area shall not be <br />altered or modified by cutting, filling, or other developmental activities which would impede the <br />successful operation of the initial or replacement soil absorption systems. Failure to comply with <br />the above items may void this approval. <br /> <br />Th~s report is not a permit to install the sewage system, A permit must be obtained before <br />any system construction occurs, A permit may be issued only after the property has been found to <br />be compatible with applicable LCDC acknowledged local comprehensive land use plan. A Statement of <br />Land Use Compatibility or ~ts equivalent is required before a construction permit can be issued. <br /> <br />A detailed site plan, drawn to scale (ex: 1"= 60"), must be submitted with the permit <br />application before the construction-installation permit can be issued. The system must be <br />installed by the property owner or a registered contractor, <br /> <br />This site evaluation report is valid until an on-site sewage system is installed pursuant to the <br />construction permit obtained from this office, or until cancelation pursuant to the Environmental <br />Quality Commission Rules. THIS SEPTIC REVIEW WAS COiqPLETED BY LEE GRIMES ON MAY 15. <br /> 1998. SOIL CONDATIONS AT THE TEST PITS APPEAR MARGINALLY <br /> SUITABLE FOR SAND FILTER PLUS FULL CAPPING FILL DRAINFIELDS. <br /> DRAINFIELDS MUST BE INSTALLED ON THE HIGHEST LANDFORM WITHIN <br /> THE IMMEDIATE AREA OF THE TEST PITS. TEST PIT SITES MUST BE <br /> BE STAKED FOR FUTURE LOCATIN IF BACKFILLED FOR SAFETY. NOTE. <br /> THE DRY SOIL. SUMMER SEASON CONSTRUCTION REQUIREMENT FOR THE <br /> CAPPING FILL DRAINFIELD INSTALLATION. OBSERVE ALL USUAL SET- <br /> BACK AND SEPARATION DISTANCES. USE CARE NOT TO ALTER THE TOP <br /> SOIL WITHIN THE DRAINFIELD SITES PRIOR TO INSTALLATION. <br />2. THE A~ACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an ~nspection, The card must be located <br /> near the driveway to the property to assist the ~nspector in <br /> locating the access onto the site, <br />3, THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities.
