Based on the results of this stuUy, <br />site approval has been granted, This approval is specific to the area tested, and the system <br />must be installed in the imediate area of the test pits, <br /> <br />Soils in the approved area shall not be altered or modified by cutting, filling, or other <br />activities which would impede the successful operation of the septic system. Observe all <br />required setback/separation distances during the installation of the system, <br /> <br />Keep this document with your records. Give a copy of this permit to the septic system <br />installer prior to the installation of the system, Any deviation from the approved plan <br />must have prior approval from this office. <br /> <br />The following construction specifications shall apply to the installation of the septic system: <br /> <br />TYPE OF REPLACEMENT SYSTEM: STANDARD <br /> <br />MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS <br /> LINEAL FEET OF DRAINFIELD: 45D <br /> TYPE OF DISTRIBUTION: SERIAL <br /> DISPOSAL TRENCH DEPTH: 24INCHES. MININUM; <br /> TOTAL DP~4IN MEDIA DEPTH: 12INCHES <br /> DRAIN MEDIA UNDER PIPE: 6INCHES <br /> MINIMUM SOIL BACKFILL: 12INCHES <br /> GROUND WATER INTERCEPTOR: Y <br /> <br />24INCHES. MAXIMUM <br /> <br />The disposal trenches are constructed two (2) feet wide and on minimum ten (10) foot <br />centers and with no line exceeding 125 feet in length. The trenches and perforated <br />pipe are installed level (+/- one inch) and contoured to follow the natural ground <br />surface. <br /> <br />COMMENTS ARD CONDITIONS <br /> <br />1, CALL BEFORE YOU DIG .... THE A1-FACHED ORANGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection. The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site, <br />4, THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any <br /> vehicular traffic, livestock and farming activities, Other <br /> activities such as logging, excavating or filling, driveway <br /> placement or any similar activities may also damage the soil <br /> qn the area and adversely affect the septic system approval. <br /> The areas of the initial septic system and the future repair <br /> system must both be protected. <br />5, THIS SYSTEM MUST BE INSPECTED AND APPROVED PRIOR TO BACKFILL <br /> When work is complete and ready for inspection, call our <br /> Automated Inspection Request line at 373-4427 to request an <br /> inspection, Enter the number "801" for a PRE-COVER inspec- <br /> tion, You may leave a message for the inspector if desired, <br /> Be sure the System Record form with the As-Built drawing is <br /> on the job or in our office when the inspection is requested <br /> An inspection should be completed within seven (7) days, <br />6, A CURTAIN DRAIN IS REQUIRED as part of this system, The <br /> curtain drain trench must be constructed a mimimum twelve <br /> (12) inches wide by 42 inches deep with 30 inches <br /> of clean gravel placed over a four (4) inch smooth wall, <br /> perforated pipe, The curtain drain must be located at least <br /> <br /> <br />
