Based on the results of this study,
site approval has been granted. This approval is specific to the area tested, and the system
must be installed in the immediate area of the test pits,

Soils in the approved area shall not be altered or modified by cutting, filling, or other
activities which would impede the successful operation of the septic system. Observe alt
required setback/separation distances during the installation of the system.

Keep this document with your records. Give a copy of this permit to the septic system
installer prior to the installation of the system. Any deviation from the approved plan
must have prior approval from this office.

The following construction specifications shall apply to the installation of the septic system:
TYPE OF REPLACEMENT SYSTEM: SAND FILTER

MINIMUM SEPTIC TANK CAPACITY: GALLONS
LINEAL FEET OF DRAINFIELD:
TYPE OF DISTRIBUTION: SERIAL
DISPOSAL TRENCH DEPTH: 12INCHES. MININUM; 12INCHES. MAXIMUM
TOTAL DI~qIN MEDIA DEPTH: 12INCHES
DRAIN MEDIA UNDER PIPE: 6INCHES
MINIMUM SOIL BACKFILL: 16INCHES
GROUND WATER INTERCEPTOR: N

The disposal trenches are constructed two (2) feet wide and on minimum ten (10) foot
centers and with no line exceeding 125 feet in length, The trenches and perforated
pipe are installed level (+/- one inch) and contoured to follow the natural ground
surface.

COMMENTS AND CONDITIONS

1. CALL BEFORE YOU DIG .... IT'S THE LAW.
A~ENTION: Oregon law requires you to follow rules adopted
by the Oregon Utility Notification Center. Those rules are
set forth in OAR 952-001-0010 through OAR 952-001-0090. You
may obtain cop~es of the rules by calling the center.
(Note: The telephone number for the Oregon Utility Notifica-
tion Center is (503) 222-1987),
2. THIS SEPTIC REVIEW WAS COMPLETED BY LEE GRIMES ON MAY 4.
1999, SOIL CONDITIONS AT THE TEST PITS ARE TOO SHALLOW TO
BASALT BEDROCK TO CONSIDER ANY ON-SITE SEWAGE DISPOSAL OTHER
THAN AN ALTERNATIVE SAND FILTER PLUS FULL CAPPING FILL DRAIN
FIELD. SAND FILTER PLANS ARE NECESSARY BEFORE THE REPAIR
PERMIT CAN BE ISSUED.
3. THE AI~FACHED OPd~NGE CARD MUST BE POSTED AT THE COUNTY ROAD
prior to requesting an inspection. The card must be located
near the driveway to the property to assist the inspector in
locating the access onto the site, '
4. THE APPROVED SEPTIC SYSTEM AREA MUST BE PROTECTED from any
vehicular traffic, livestock and farming activities. Other
activities such as logging, excavating or filling, driveway
placement or any similar activities my also damage the soil
in the area and adversely affect the septic system approval.
The areas of the initial septic system and the future repair
system must both be protected.
5, SAND FILTER systems are installed in soils where a layer
that limits the effective soil depth, or a water table is
present in the soil profile at a depth much shallower than
allowed for a standard system. The sand filter unit
pre-treats the septic tank effluent before it is discharged
into the ground, The drainfield following the sand filter
is installed at a very shallow depth and generally includes
a capping fill drainfield. See the attached "Sand Filter
System Requirements for Plans". Two sets of plans for the
system must be submitted at the time of application for the
