Based on the results of this study, site <br />approval was granted. The approval is site specific to the area tested, and the septic <br />system must be located in the immediate area of the test pits, unless otheowise authorized <br />by this office, A new site evaluation may be required if the system is proposed for a <br />different area on the property. <br /> <br />This approval was given on the basis that the lot or parcel will not be further partitioned <br />or subdivided and that conditions on this property and on adjacent properties are not altered <br />in any manner wlich would prohibit the issuance of a permit. Soils in the approved area <br />shall not be altered by cutting, filling, or other developmental activities which would <br />impede the successful operation of the initial or replacement absorption systems. Failure <br />to comply with the above items may void this approval. <br /> <br />The site evaluation approval runs with the land, but the permit is non-transferable and non- <br />refundable. <br /> <br />Keep this document with your records. Show this permit to the septic installer prior to <br />installation of the system. THIS SEPTIC REVIEW WAS COMPLETED BY Z.EWING ON 5/19/00. THE <br /> SITE IS APPROVED FOR AN ALTERATION OF THE CAPPING FILL DRAIN <br /> FIELD WHICH WAS INSTALLED IN APRIL 1997. THE PROPOSAL IS TO <br /> ELIMINATE A PORTION OF THE EXISTING DRAINFIELO ON THE EAST <br /> SIDE AND REPLACE THAT WITH A LIKE AMOUNT OF DRAINFIELD LINE <br /> AS SHOWN ON THE ATTACHED PLAN. THIS CAPPING FILL MUST BE IN- <br /> STALLED DURING THE DRY SUMMER SEASON AND THE CAP INSTALLED <br /> AS PER APPROVED PROCEEOURE. <br />3. THE ATTACHED DPJ~NGE CARD MUST BE POSTED AT THE COUNTY ROAD <br /> prior to requesting an inspection. The card must be located <br /> near the driveway to the property to assist the inspector in <br /> locating the access onto the site. <br /> <br /> <br />
