Based on the results of this study, site approval was granted. The approval is site specific to the area tested, and the septic system must be located in the immediate area of the test pits, unless otheowise authorized by this office, A new site evaluation may be required if the system is proposed for a different area on the property. This approval was given on the basis that the lot or parcel will not be further partitioned or subdivided and that conditions on this property and on adjacent properties are not altered in any manner wlich would prohibit the issuance of a permit. Soils in the approved area shall not be altered by cutting, filling, or other developmental activities which would impede the successful operation of the initial or replacement absorption systems. Failure to comply with the above items may void this approval. The site evaluation approval runs with the land, but the permit is non-transferable and non- refundable. Keep this document with your records. Show this permit to the septic installer prior to installation of the system. THE SITE IS APPROVED FOR AN ALTERATION OF THE CAPPING FILL DRAIN FIELD WHICH WAS INSTALLED IN APRIL 1997. THE PROPOSAL IS TO ELIMINATE A PORTION OF THE EXISTING DRAINFIELO ON THE EAST SIDE AND REPLACE THAT WITH A LIKE AMOUNT OF DRAINFIELD LINE AS SHOWN ON THE ATTACHED PLAN. THIS CAPPING FILL MUST BE IN- STALLED DURING THE DRY SUMMER SEASON AND THE CAP INSTALLED AS PER APPROVED PROCEEOURE. 3. THE ATTACHED DPJ~NGE CARD MUST BE POSTED AT THE COUNTY ROAD prior to requesting an inspection. The card must be located near the driveway to the property to assist the inspector in locating the access onto the site.
