Please read the permit carefully to become familiar with the <br />terms and conditions to which you must comply. Failure to comply with all conditions in <br />the permit may result in enforcement action, including civil penalties. <br /> <br />Schedule B of tho permit requires that an annual report be submitted addresdng the <br />required monitoring, operation and maintenance activities. This report must be submitted <br />to the Department by January 15ts of each ye&r. There is an annual fee associated with this <br />permit. The first year's Annual Compliance Determination fee has been submitted as part <br />of your application fees. Your annual fee is currently $500.00. ' <br /> <br />Your permit is issued under the following conditions: <br /> <br />1. The system is to serve aircraft hangers and aircrffi related commercial/industrial <br /> facilities wtth one c~rctaker s residence. <br /> <br />2. The maximum daily sewage flow shall not exce~ 4,999 gallons on any tingle day. <br /> The system is not designed to treat sewage effluent flows of 4,999 gallons on a daily <br /> basis. The system is designed to treat approximately 2,500 gallons of sewage effluent <br /> on a daily basis. <br /> <br /> <br />
