The plan is dated <br />and signed by Randall Trox. <br /> <br />Thc final treatment and disposal will be aceompligaed by mca~s of a sewage treatment <br />and disposal drainfleld. The diainfield shall be i~stalled at a minimum size of 1,499 <br />linear feet and in.tailed in accordance with the approved construction plan. <br /> <br />5. All septic t..~', and pump tanks shall be tested for water tighmess in accordance with <br /> OAR 340-73-25(3), and found to be water tight before using. <br /> <br />Prior to connection to and use of the system, the installer, in cooperation with <br />Environmental Management Services, Lac. shall certify in writing, using the FINAL <br />INSP~'TION REQUEST AND NOTICE form to the Department, that all aspects of <br />the construction are complete and correct. <br /> <br />By no later than one month after commencing operation of the system, Environmental <br />Management Services, Inc. shall submit to the Department written affirmation that the <br />system is operating as intended. <br /> <br />8.
