Based on the results of this study, site evaluation <br />approval is granted. The approval is site specific to the area tested, and the septic system <br />must be located in the immediate area of the test pits, unless otherwise authorized by this <br />office, A new site evaluation may be required if the system is proposed for a different <br />area on the property, <br /> <br />This approval is given on the basis that the lot or parcel will not be further partitioned <br />or subdivided and that conditions on this property and on adjacent properties are not altered in <br />any manner which would prohibit the issuance of a permit. Soils in the approved area shall not be <br />altered or modified by cutting, filling, or other developmental activities which would impede the <br />successful operation of the initial or replacement soil absorption systems. Failure to comply with <br />the above items may void this approval. <br /> <br />This report is not a permit to install the sewage system. A permit must be obtained before <br />any system construction occurs. A permit may be issued only after the property has been found to <br />be compatible with applicable LCDC acknowledged local comprehensive land use plan. A Statement of <br />Land Use Compatibility or its equivalent is required before a construction permit can be issued, <br /> <br />A detailed site plan, drawn to scale (ex: 1''= 60"), must be submitted with the permit <br />application before the construction-installation permit can be issued. The system must be <br />installed by the property ovmer or a registered contractor, <br /> <br />This site evaluation report is valid until an on-site sewage system is installed pursuant to the <br />construction permit obtained from this office, or until cancelation pursuant to the Environmental <br />Quality Commission Rules, Subject to the above, this report runs with the land and will automat- <br />ically benefit subsequent owners. <br /> <br />Site conditions will require the following installation specifications: <br /> <br />TYPE OF INITIAL SYSTEM: STANDARD <br /> <br />MINIMUM SEPTIC TANK CAPACITY: 1000 GALLONS <br />LINEAL FEET OF DRAINFIELD: 450 <br />TYPE OF DISTRIBUTION: SERIAL <br />DISPOSAL TRENCH DEPTH: 24INCHES, MINIMUM; 24INCHES.
