STATE OF OREGON <br />DEPARTMENT OF ADMINISTRATIVE SERVICES <br />PURCHASING DIVISION <br />PRICE AGREEMENT SUMMARY <br />PAGE: 8 <br />REVISION <br />NUMBER: 002 <br />COMMODITY CODE: 42560 PA <br />NUMBER: 7119 <br />and <br />agency <br />of <br />TRAINING: In addition to individually tailored measurement, <br />fitting, delivery, assembly and installation, Contractor shall <br />provide on-site feature and operational training to each user. <br />Further, for all chairs, Contractor shall supply in written <br />format, at no additional cost to the State, training which <br />provides complete, step-by-step, instructions in the total use <br />operation of the chair. In addition to the written format, <br />Contractor may also provide the instructions on disc (ASCII <br />FORMAT), if available. Contractor shall also provide the <br />with the name and telephone number of a contact person who can <br />provide, over the phone, instructions in the use and operation <br />the chair. This information shall be supplied by Contractor at <br />time of delivery. <br />CATALOGS: Contractor shall, upon request, provide <br />manufacturer's <br />catalogs and price lists which reflect the chairs identified in <br />the price agreement to the State. Contractor shall update the <br />material as necessary. <br />PERFORMANCE: Contractor shall ensure that all products <br />supplied <br />under this contract perform satisfactorily and that all <br />services <br />are accomplished in the most highly professional manner. Al1 <br />goods and services established under this contract shall be <br />provided to State agencies and all authorized ORCPP members. <br />With <br />
