The tenn "fnnge benefits" refers to F~ayiii~=r~~ ; <br />si3ch as: <br />~) medical or hospital r.are; pensions on reUrement or death; compensation for ii~~uries or illness <br />resulting fr~m an occiipatior~al activity, or insurance to provide airy of the foregoing; <br />b) unemployment benefits, life insurance, disability and sickness ir7s~irance or accident insui~~nce; <br />c) vacation and holiday pay; <br />d) defraying costs of aE7prenticeship or other similar programs; and <br />e) other such bona fide benefits. <br />For the purpose oi prevailing wac~c~ rates, frin~3e benefits ~~~ include any ber7etits which may be requ~red by <br />federal, state c~r local law (e.g. Workers' Compensation, Unemployment Insura~ice, etc.). <br />Every contractor or subcontractor th~t provldes for or contributes to a health and weltare plan or a pension plan, <br />rnust post notic~ c~escribing such plans iri a conspicuous and accessible place crn the project.(ORS 279.350(51J <br />Fringe benefits inay t~e paid to the worker in cash or to a third party administeririg a fringe benefit proyram. T~his statement must completely and ~~ccurately refler.t payroll records f~~r the <br />work week immediately precediny the submission. A cc>ntr'actor or subcontr~~~ctor must complete and subinit th~? <br />certified statement contained on Fc~rrn WH 38 as well as the intormatiori r~~quirecl on the weekly ~~ayroll. Forrn <br />WN-38 and instructions for cornpleting it are included in this booklet. The schedule for submitting payr~~ll <br />iniormatfon is as follows: Once within 15 d~~ys of the date the contractor oi subcc~ntractor first began work on the <br />project; once before the final inspection of the project by the public contracting agency; in addition, f91 R141C~t~ <br />~X~~~S~ii19.~4_S~s3y&, submissions are to be made at 90 day intervals. WFien work on a project starts and finishes in <br />15 days ar less, the contractar ar subr,ontractor which performed the work shall submit a payroll and certified <br />statement form which accurately and cornpletely sets out the payroll for all the work perfonned on the project. <br />Payroll information is to be filed with the public contracting agency. 7he payroll information must be kepl by the <br />contractor and or subcc~ntractor for tf~ree years. <br />
