Contractor shall provide written violation notices.to program participants who accumulate two no-show <br />~ violations within 30 days explaining that two additional no-shows within the following 30 days will <br />result in a suspension of service for at least 10 days. CONTRACTOR shall send the written <br />notification to the program participant within 24 hours after the second no-show. CONTRACTOR will <br />provide program participant written notification of any and all suspensions which shall include notice to <br />the availability of an appeal. The DISTRICT will not reimburse CONTRACTOR for a trip provided to a <br />program participant who has more than four no-shows within 30 days, exclusive of no-shows beyond <br />the program participants' ability to control. <br />2.11 One-way Trip Definition <br />A trip is defined as authorized travel befinreen two points. A trip is authorized by the CONTRACTOR's <br />dispatch personnel and requires an authorization number assigned by the dispatcher. Program <br />participants must request the ride through the dispatcher by making the request by phone. Stop-over, <br />route deviation and/or additional trips requested by the rider are considered an additional trip and <br />require authorization though the dispatch procedure. The DISTRICT will not reimburse for <br />unauthorized trips. <br />2.12 Trip Log <br />CONTRACTOR's drivers are required to log the program participant's ID number, trip origin and <br />destination, fare or other revenue collected, the time of day that trip begins and ends, and the total trip <br />mileage. CONTRACTOR will provide DISTRICT with the necessary trip log sheets and surveys and <br />assist by randomly recording selected trip surveys that are consistent with the National Database <br />Program (formerly Section 15) requirements. <br />2.13 Trip Posting <br />Posting is the procedure whereby the information supplied by the driver from the trip log is matched to <br />the scheduled reservation information supplied ftom the dispatcher's trip log. Proper and timely <br />posting is necessary to ensure the DISTRICT can prepare and process invoices in a timely fashion. All <br />trip log posting will take place within seven days following provision of services. <br />2.14 Passenger Assistance <br />CONTRACTOR is required to use the highest degree of safety in the operation of equipment and <br />assistance of riders. CONTRACTOR shall ensure that program participants are secured with fastened <br />safety belts and ensure that wheelchairs/scooters are secured with a 4-point tie down system. <br />CONTRACTOR shall prohibit drivers from physically lifting passengers from scooters to seats, and <br />lifting wheelchairs/scooters into paratransit vehicles. <br />2.15 Smoking/Eating/Drinking <br />(a) CONTRACTOR will ensure that smoking, eating and consumption of beverages <br />by drivers or passengers while on-board any vehicle engaged in performing service for the DISTRICT <br />is not permitted. <br />109 <br />I CHERRYLIFTCONTRACT AGREEMENT#1296.LIFf <br />
