~ ~ <br />Entry and Cleaning" <br />Publication 2201: "Procedures for Welding or Hot Tapping on Equipment <br />Containing Flammables" <br />Publication 2202: "Dismantling and Disposing of Steel from Tanks Which Have <br />Contained Leaded Gasoline" <br />Publication 2209: "Pipe Plugging Practices" <br />1.4 SUBMITTALS <br />A. A project involving the removal of underground storage tanks (LJSTs) must be <br />completely and accurately documented to ensure proper project closure with the <br />DEQ. Marion County, or its designated representative, will prepaze a final project <br />report describing the UST removal. The Contractor shall assist in this effort by <br />providing to the Engineer such information or documentation specified in Post- <br />Removal of Tank below. <br />B. Submit a written work plan giving a detailed account of how Contractor intends to <br />conduct the excavation and removal of the tank or tanks at each site. As a <br />minimum, the plan shall include the following: <br />1. A discussion of proposed excavation methods. <br />2. A copy of the Contractor's Health and Safety Plan in accordance with <br />OSHA and DEQ requirements. <br />3. Evidence of licensing with the DEQ according to the provisions of OAR <br />Chapter 340, Division 160 for the firm and individuals responsible for <br />supervising work at each project site. <br />4. A discussion of procedures, locations, and methods to be used for <br />providing and maintaining temporary on-site stock piles of excavated <br />materials for each site. <br />5. List the name and address of the proposed disposal facility for <br />contaminated water and tank contents. <br />6. Submit a copy of "The UST Decommissioning/Change-In-Service Report <br />Form." <br />7. Submit a copy of "The UST Decommissioning Form." <br />1.5 CONTRACTORS QUALIFICATIONS <br />A. Underground storage tank removal and disposal shall be accomplished by a firm <br />Excavation, Removat, Decontamination and Disposal of USTs 02085-3 <br />
