The pertinent information includes the following: <br />1) Occupant Interview: Mr. Heinlc occupied the Senator Building in September I996. The <br />rtnt was 5275 per month plus S24 per month electricity. The Occupant Interview form was <br />not signed or dated by the relocation agent. No other party was listed as an occupan[. <br />2) Replacement Housing Payment DetErminaaon: A housing study was made and a <br />determination of benefits was made on Febniary 27, 1997 by Becca Reel. Ba.sed on <br />Comparable A+1, the rent supplement was calculated to be 5462.00. There was no application <br />of the 30~ rule for gross monthly income. <br />3) Relocation Benefit Summary: Mr. Heinle was given a benefit summary on February 27, <br />1997 which identifiod his monetary relocation benefits as $550.00 for a sel~ move and <br />a4b2,00 for a rent supplement. <br />4) 90-d2y nodce: Mr. Heinle was given a 90 day notice to vacate on February 27, 199'7. <br />S} Schedule Move Claim Form: Mr. Heinle was given a schedule move claim from for <br />SSS0.00 on April 18, 1997. <br />6) Replacement Housing Claim Form: Mr Heinle was given a replacement housing claim <br />form on July 17, 1997 for SS 17.00 which included $462.00 for rent supplement and 555.00 <br />for a non-refundable deposit. <br />H~NN~ h~1CELD0~,~1NEY <br />~ Replac,~ment Dwelling Inspe~ction Form: Becca Reel and Eric Heinle signed a replacement <br />dwelling inspection form on July 17, 1997.
