Pernut Number: 1500A <br />Expiration Date: 6/30/2000 <br />Page 10 of 10 Pages <br />10. <br />il <br />CFR 122.22. <br />~ <br />Falsification of Reports <br />Under ORS 468.953, any person who lrnowingly makes any. false statement, representation, or certificarion in any <br />record or other document submitted or required to be matntained under this permit, including monitoring reports or <br />reports of compliance or noncompliance, is subject to a Class'C felony punishable by a fine not to exceed 5100,000 per <br />violation and up to 5 years in prison. <br />Chanees to Indirect Dischar~ers -[Applicable to Publicly Owned Treatment Works (POTV-~ only] <br />The permittee must provide adequate notice to the Department of the following: <br />a. Any new introduCrion of pollutants into the POTW from an indirect discharger which would be subject to <br />section 301 or 306 of the Clean Water Act if it were directly dischar~ing those pollutants and; <br />b. Any substantial change in the volume or character of pollutants being introduced mto the POT'W by a source <br />introducmg pollutants mto the POTW at the time of issuance of the permi~ <br />c. For the purposes of this para~a h, adequate notice shall include information on (i) the quality and quantity of <br />:::lueai 's.ztr~u~e3 u:to the . That any activity has occurred or will occur which would result in the discharge, on a routine or frequent basis, <br />of any, toxic pollutant which is not limited in the permit, if that discharge will exceed the highest of the <br />followmg "norificarion levels: <br />1 One hundred micrograms per liter (100 mg/1 ; <br />2 Two hundred micro~ ams per liter (200 m) for acrolein and acrylonitrile; five hundred micrograms <br />~~ <br />per liter (500 mg/1) for 2,4-dinitrophenol and for 2-methyl-4,6-dinitrophenol; and one milligram per <br />liter (1 mg/1) for antimony; <br />(3) Five (S) tunes the maximum concentration value reported for that pollutant in the permit applicati~^ , <br />in accordance with 40 CFR 122.21(gx7); or ~ <br />(4) The level established by the Departinent in accordance with 40 CFR 122.44(~. <br />b. That any activity has occurred or will occur which would result in any discharge, on a non-routine or <br />infrequent basis, of a toxic pollutant which is not limited in the permit, if that discharge will exceed the highest <br />of the following "notification levels": <br />(1) Five hundred micrograms per liter (500 mg/l); <br />(2) One millignm per lrter (1 mg/1) for antimony; <br />(3) Ten (10) tunes the maximum concenri~ation value reported for that pollutant in the permit application <br />in accordance with 40 CFR 122.21(gx7); or <br />(4) The level established by the Department in accordance with 40 CFR 122.44(~. <br />SECTION E. DEFINITI01~1S <br />1. <br />2. <br />3. <br />4. <br />5. <br />6. <br />7. <br />8. <br />9. <br />10. <br />11. <br />12. <br />13. <br />14. <br />15. <br />16. <br />17. <br />BOD means f ve-day biochemical oxyg~n demand. <br />TSS means total suspended solids. <br />mg/1 means milligrams per liter. <br />kp meaas kilograms. <br />m/d means cubic meters per day. <br />MGD means million gallons per day. <br />Composite sample means a sample formed by collecting and mixing discrete samples taken periodically and based on <br />rime or flow. <br />FC means fecal coliform bacteria. <br />Technology based permit effluent limitations means technology-based treatrnent requirements as defined in 40 CFR <br />125.3, and concentiation and mass load ef~luent limitations that are based on minimum design criteria specified in OAR <br />340-41. <br />CBOD means five day carbonaceous biochemical oxygen demand. <br />Grab sample means an individual discrete sample coIlected over a period of time not to exceed 15 minutes. <br />Quarter means January through March, April through June, July through September, or October through December. <br />Month means calendar month <br />Week means a calendar week of Sunday throagh Saturday. <br />Total residual chlorine means combined chlorine forms plus free residual chlorine. <br />The term "bacteria" includes but is not limited to fecal coliform bacteria, total coliform bacteria, and E. coli bacteria. <br />POT'W means a publicly owned treatment works. <br />. ~ <br />
