~ <br />centurywest <br />ENGINEERING CORPORATION <br />January 12, 1998 <br />Mr. Kelly Odell <br />Gelco Services, Inc. <br />1746 Salem Industrial Dr. NE <br />Salem, OR 97303 <br />RE: Sewer Laterals Abandonment Exceptions <br />Courthouse Square Demolition Project <br />Dear Kelly: <br />The purpose of this letter is to advise you of the exceptions identified by the City of Salem when <br />they performed a TV inspection on the sewer laterals abandoned by Gelco on the Courthouse <br />Square project. The work performed by Gelco took place on December 29, 1997 from 6:00 PM <br />until approximately 2:00 AM. Gelco advised they had chemically grouted/sealed seventeen (17) <br />4"-6" service laterals connected to the 400 linear foot section of 10" diameter sanitary sewer <br />mainline identified on the subject property. The City of Salem noted five exceptions in the work, <br />please see the attachment entitled Sewer Main TV Inspection Detail prepared by the City of <br />Salem, dated 1/9/98. <br />No chemical grout was found by the City of Salem at stations 110' and 300'. A small amount of <br />grout was found at stations 108', 126', and 371'. The noted exceptions need to be corrected as <br />soon as possible. Please notify me of the schedule to complete this work. The City of Salem has <br />requested notification upon completion so that they may reinspect the sewer main. <br />Thank you for the opportunity to work with you on this project.
