May organize and facilitate care <br />coordination teams. Performs counseling and case management functions with consumers and their <br />families with clinical supervision. Keeps records of consumer status. May develop and implement specific <br />services in the community. Performs related duties as required. May be required to facilitate or coordinate <br />24/7 on call services. <br />SUPERVISION RECEIVED <br />Works under supervision of a Clinical Supervisor and/or designee who assigns work, establishes goals <br />and reviews results obtained for overall effectiveness. Works within a team and collaborates with other <br />team members. May also work independently in providing program services. <br />SUPERVISION EXERCISED <br />Supervision of others in not a responsibility of positions in this classification, however, may exercise lead <br />duties and/or responsibilities for employees, teams, or projects. <br />EXAMPLES OF DUTIES - DUTIES INCLUDE, BUT ARE NOT LIMITED <br />TO THE FOLLOWING: <br />1.Plans and implements counseling activities with other drug and alcohol health professionals in <br />providing group and individual counseling with consumers enrolled in drug and alcohol services, <br />and their families. <br />2.Performs primary case management, under supervision, to determine consumer eligibility, service <br />needs and to obtain resources for consumers. Monitors services provided to consumers. <br />3.Participates in intra-agency and interagency case staffing to assist with the evaluation and <br />treatment planning for consumers including crisis management. Coordinates with other agencies in <br />program delivery. <br />4.Acts as a facilitator or co-facilitator in group counseling sessions with substance-related disorder <br />consumers. <br />5.Serves as a member of internal and community-based program teams. <br />6.May develop and implement specific services in the community. <br />https://agency.governmentjobs.com/marion/default.cfm?action=specbulletin&ClassSpecID=...8/3/2020 <br /> <br />
