It <br /> consists of a 1500-gallon two-compartment combo tank, a Sand Filter <br /> treatment unit and 177 lineal feet of drainfield line. The sand filter is failing and <br /> must be replaced. The other components will remain in use. <br /> This approval is for a system designed to serve a single family dwelling with a <br /> maximum of four bedrooms. Peak sewage flow into the system is limited to a <br /> maximum of 450 gallons per day, with an average sewage flow of not more <br /> than 225 gallons per day. <br /> The plan for installation of the treatment'unit is approved as REVISED. Please <br /> refer to the signed approved site plan dated February 5, 2019. Any deviation <br /> from the approved plans must be authorized by this office prior to installation or <br /> the permit may be voided. It is the responsibility of applicant/installer to <br /> coordinate these requirements with all others in OAR 340-071-0220. <br /> SPECIFIC REQUIREMENTS <br /> ATT Pretreatment Unit: MicroFAST 0.5 as per approved attached plans. <br /> SERVICE CONTRACTS: The owner of an ATT system must maintain a <br /> service contract with a maintenance provider certified by the manufacturer to <br /> serve and maintain the onsite system. A service contract must be entered into <br /> before the system is installed and must be maintained until the system is <br /> decommissioned. The service contract must provide for a minimum of four <br /> inspections and service visits by a maintenance provider scheduled once every <br /> six months over a two-year period to inspect, adjust, and service the ATT. The <br /> septic tank must be pumped before solids reach 30% of the tank liquid <br /> operating level. The maintenance provider must submit an annual report and <br /> fee. Please review OAR 340-071-0345 for full requirements.
